Unilateral Dudak-Damak Yarıklı Erişkin Hastada Estetik Tedavi: Olgu Sunumu

Abstract

Dudak damak yarıkları (DDY) embriyonik ve erken fetal dönemde oluşan, gelişim varyasyonlarının sonucu olarak gelişmektedir. DDY'li hastalar ilerleyen yaşamları boyunca sosyal, fonksiyonel ve psikolojik problemlerle karşılaşırlar. Bu olgu sunumunun amacı, estetik beklentisi yüksek unilateral DDY'li bir hastanın protetik tedavisini sunmaktır. Yirmi bir yaşında dudak damak yarıklı erkek hasta Marmara Üniversitesi Protetik Diş Tedavisi Anabilim Dalı Kliniği’ne ön dişlerinin estetik olmayan görüntüsü nedeniyle başvurdu. Hastanın yapılan klinik ve radyografik muayenesinde maksiller sol santral ve lateral dişler arasında yarık hattının mevcut olduğu gözlendi. Ortodontik tedavi görmesine rağmen yarık hattının varlığı sebebiyle 21 ve 22 nolu dişler arasında 8 mm’lik diastema mevcut olduğu tespit edildi. Bu nedenle estetik problemler yaşayan hastanın 11 ve 12 nolu dişine laminate veneer ve 21-22 ve 23 nolu dişlere 4 üyeli zirkonyum alt yapılı seramik köprü planlandı. Tam seramik restorasyonlar adeziv rezin siman ile simante edildi. Sabit tedavi sonrası, ideal estetik görünüş, fonksiyon, stabilite ve optimum fonasyon sağlandığı gözlendi. Dudak damak yarığı hastalarında erişkin dönemde uygulanan bir çok daimi protetik rehabilitasyon yaklaşımı mevcuttur. Tam seramik sistemlerle planlanan sabit köprü protezler yüksek estetik sonuçlar vermektedir.

Aesthetic Rehabilitation for an Adult Patient with Unilateral Cleft Lip and Palate: Case Report

Abstract

Cleft lip and palate is one of the most common congenital malformations affecting the face and the jaws. These patients suffer from aesthetic, functional, social and psychologic problems. The objective of this case report is to present the prosthetic rehabilitation of the patient with unilateral cleft lip and palate, and highly aesthetic expectation. A 21-year-old patient with cleft lip and palate was referred to Marmara University Prosthesis Department due to the non-aesthetic appearance of the anterior teeth. In clinical and radiographic examination, it was observed that there was a cleft line between the maxillary left central and lateral teeth. Although there is orthodontic treatment, there is an 8 mm diastema between the teeth 21 and 22 due to the presence of the cleft line. For this reason, zirconium ceramic bridge with four members was planned to teeth 21-22 and 23 and with laminate veneer 11 and 12 of the patient. Full ceramic restorations were cemented with adhesive resin cement. After fixed prosthetic rehabilitation; the patient reported that he was satisfied with the aesthetic appearance, function and fonation. There are numerous permanent prosthetic treatment alternatives for the patients with adult cleft lip and palate. Highly aesthetic results can be achieved with fixed bridge restorations with all ceramic systems.
