AMELİYAT ÖNCESİ ORAL KARBONHİDRAT SOLÜSYON KULLANIMININ AMELİYAT SONRASI İNSÜLİN DİRENCİNE ETKİSİ

Abstract

Geleneksel ameliyat öncesi bakımda, ameliyat öncesi
gece katı ve sıvı alımının kesilmesi ile mide sıvı hacminin azalacağı ve
cerrahi sırasında pulmoner aspirasyon riskini azaltacağı düşünülerek bireyler
uzun süreli açlığa maruz bırakılmaktadır. Bu uygulamayı kanıtlayan bilimsel
çalışmaların bulunmaması nedeni ile son yıllarda elektif cerrahi uygulanan
hastalarda ameliyat öncesi ve sonrası durumun optimize edilmesi amacı ile
ameliyat öncesi oral karbonhidrat solüsyonlarının kullanımı göze çarpmaktadır.
Cerrahi hastaların ameliyat öncesi refah düzeyini arttırmak için çoğu batı
ülkesi son 10-15 yıldır rutinlerini değiştirerek anesteziden 2 saat öncesine
kadar berrak sıvıların alınmasına izin vermektedir. Uygulamanın temel hedefi;
ameliyat öncesi dönemde açlık süresini kısaltarak ameliyat sonrasında stres
düzeylerinin ve travmanın neden olduğu insülin duyarlılığını azaltarak kan
şekerinin düşürülmesidir. Bu derlemede ameliyat öncesi oral karbonhidrat
solüsyonu kullanımının ameliyat sonrası insülin direnci üzerine etkisi
tartışılmıştır.
