DOĞUMUN İKİNCİ EVRESİNDE IKINMA VE IKINMA SÜRECİNDE EBENİN SORUMLULUKLARI

Abstract

Fetüsün prezente olan kısmının pelvik tabandaki sinir pleksuslarına yaptığı baskı sonucu oluşan içsel dürtüye ıkınma adı verilir. Ikınma dürtüsü genellikle doğumun 2. evresinde ortaya çıkar. Doğumda ıkınma; fetüsün pelvisten iniş süreci ile doğumda ikinci evrenin kısaltmasını ve doğumu kolaylaştırmayı sağlamaktadır. Valsalva (kapalı glotis) ve spontan (açık glotis) olmak üzere iki tip ıkınma mevcuttur. Valsalva tipi ıkınma, gebenin nefesi tutarak gerçekleştirdiği ıkınma tipidir. Bu ıkınma, genellikle sağlık profesyonelleri tarafından direktifli (yönlendirilmiş) ıkınma kapsamında değerlendirilmektedir. Kadının ne zaman ve nasıl ıkınacağı konusunda sağlık profesyonelleri tarafından yönlendirilmektedirler. Spontan ıkınma, gebenin ses çıkararak ve nefes vererek yaptığı ıkınma tipidir. Gebe ıkınmayı spontan olarak gerçekleştirmektedir ve ıkınma kontraksiyonun şiddetiyle uyumludur. Dünya Sağlık Örgütü, pozitif doğum deneyimi için anne ve fetüs sağlığını koruyan spontan ıkınmanın cesaretlendirilmesini, uzun süreli nefes tutularak yapılan valsalva ıkınmanın engellenmesini önermektedir. Anne adayının ıkınma etkililiğini ebe; fetüsün angajman seviyesi, servikal dilatasyon durumu, fetal prezantasyon ve uterus kontraksiyonunun şiddetini değerlendirerek yapmalıdır. Ebenin ıkınma süresince; spontan ıkınmayı destekleme, ideal ıkınma ortamı yaratma (pozisyon verme, mahremiyeti sağlama vb.), bire bir doğum desteği sağlama, empatik iletişimde bulunma gibi sorumlulukları vardır. Bu derlemenin amacı; doğumda ıkınma tipleri, ıkınmanın maternal ve fetal sağlık üzerine etkileri ve ıkınma sürecinde ebenin rol ve sorumluluklarını ortaya koymaktır.

PUSHING IN SECOND STAGE OF LABOUR AND RESPONSIBILITIES OF MIDWIFERY IN PROCESS PUSHING

Abstract

The internal impulse created by the pressure exerted by the presenting part of the fetus on the nerve plexuses in the pelvic floor is called pushing. Pushing in birth provides shortening of the second stage at birth and facilitating delivery by the process of descent of the fetus from the pelvis. There are two types of pushing, valsalva (closed glottis) and spontaneous (open glottis). Valsalva type pushing: It is the type of pushing performed by the pregnant woman by holding her breath. This pushing is generally considered as directed (directed) pushing by health professionals. Health professionals are guided by how and when a woman should bother. Spontaneous pushing: It is the type of pushing that the pregnant woman makes by making sounds and breathing. The pregnant pushing spontaneously and the pushing is compatible with the severity of the contraction. The World Health Organization recommends encouraging spontaneous pushing that protects the health of the mother and fetus for a positive delivery experience and preventing valsalva pushing by holding the breath for a long time. The midwife examines the pushing effect of the expectant mother; by evaluating fetal engagement level, cervical dilatation status, fetal presentation, and severity of uterine contraction. The midwife has responsibilities such as supporting spontaneous pushing, creating an ideal pushing environment (positioning, providing privacy, etc.), providing one-on-one birth support, and empathic communication. The purpose of this review is to reveal the types of pushing at birth, the effects of pushing on maternal and fetal health, and the roles and responsibilities of the midwife in the pushing process.
