Comparison of Sexual Function and Incontinence Scores in Cases of Burch and Transobturator Tape For Stress Urinary Incontinence

Abstract

Aim: Aim: This study aims to compare trans obturator tape (TOT) and Burch operations performed for the surgical treatment
of stress urinary incontinence in terms of postoperative residual volume, sexual function and incontinence scores.
Material and Method: The study was designed as prospective observational. Cases who underwent urinary incontinence
surgery between September 2014 and January 2018 were included. Patients who underwent trans obturatory tape and
Burch were compared in terms of postoperative residue, Arizona Sexual Experiences Scale (ASEX) and International
Incontinence Questionnaire-Incontinence Short Form (ICIQ-SF). Student t, Mann-Whitney u, Wilcoxon tests were used
for statistical analysis. P<0.05 was considered significant.
Results: The records of 52 patients who underwent stress urinary incontinence surgery, 29 of whom were TOT and 23
were Burch, were retrieved. The mean age was 55.50±9.71 years, the mean follow-up time was 14.63±9.33 months,
and the mean postoperative urine residue was 30.40±16.75 cc. There was a statistically significant difference between
the groups regarding the postoperative residual volume of urine (p<0.05). There was a significant difference between
the Burch and TOT groups in terms of mean age (53.60–57.00) and body mass index (BMI) (27.60-30.45) (p<0.05). A
statistically significant difference was found between preoperative and postoperative ICIQ-SF scores in groups (Burch
preop:11.09/ postop:1.30 - TOT preop:16.59/postop:0.79) (p<0.05). (p<0.05). Subjective clinical improvement was
recorded in more ICIQ-SF scores in the TOT group. The decrease in the postoperative ASEX score averages in the Burch
(Preop 16.96/ postop 13.04) group was statistically significant (p<0.05), while this score decreased in the TOT (preop 19
/ postop 18.24) group after the operation.
Conclusion: While there was a significant improvement in sexual functions in the cases whom Burch was applied, a
significant improvement was also found in the incontinence scores of the issues in which TOT was used.

Stres Üriner İnkontinans Nedeniyle Burch ve Transobturator tape Uygulanan Olgularda Seksüel Fonksiyon ve İnkontinans Skorun Karşılaştırılması

Abstract

Amaç: Bu çalışmanın amacı stres üriner inkontinansın cerrahi tedavisinde uygulanan trans obturatuar tape (TOT) ve Burch
operasyonlarını postoperatif rezidüel volüm, seksüel fonksiyon ve inkontinans skorları açısından karşılaştırmaktır.
Gereç ve Yöntem: Çalışma prospektif gözlemsel olarak dizayn edildi. Eylül 2014- Ocak 2018 tarihleri arasında üriner
inkontinans cerrahisi uygulanan olgular dahil edildi. Trans obturatuar tape ve Burch uygulanan hastalar postoperatif
rezidü miktarı, Arizona Sexual Experiences Scale (ASEX) ve International Incontinence Questionnaire-Incontinence Short
Form (ICIQ-SF) açısından karşılaştırıldı. İstatistiksel analiz için student t, mann-whitney u, wilcoxon testleri uygulandı.
P<0,05 anlamlı kabul edildi.
Bulgular: 29'u TOT, 23'ü de BURCH olmak üzere toplam 52 stress üriner inkontinans cerrahisi uygulanan olgu çalışmaya
dahil edildi. Yaş ortalaması 55,50±9,71, ortalama izlem süresi 14,63±9,33 ay ve ortalama postoperatif rezidü miktarı
30,40±16,75 cc idi. Gruplar arasında post operatif rezidüel volüm idrar miktarı açısından istatistiki olarak anlamlı farklılık
vardır (p<0,05). BURCH ve TOT grupları arasında yaş (53,60–57,00) ve vücut kitle indeksi (VKI) (27,60-30,45) ortalamaları
yönünden anlamlı farlılık vardı (p<0,05). ICIQ-SF skorlarında ameliyat öncesi ve sonrası arasında istatistiki olarak anlamlı
farklılık bulunmuştur (BURCH preop:11,09/postop:1,30 - TOT preop:16,59/postop:0,79) (p<0,05). BURCH grubunda
operasyon sonrası ASEX skor ortalamalarındaki azalma (Preop 16,96 / postop 13,04) istatistiki olarak anlamlı bulunmuştur
(p=0,03), TOT grubunda operasyon sonrası ASEX skorunda (preop 19 / postop 18,24) azalma görülmüştür.
Sonuç: BURCH uygulanan olgularda seksüel fonksiyonlarda anlamlı olarak düzelme tespit edilmişken TOT uygulanan
olgularda ise inkontinans skorlarında anlamlı düzelme tespit edilmiştir.
