Jinekolojik laparoskopinin anahtar enstrümanları: Uterin manipülatörler

Abstract

Uterin manipülatörler, ileri seviyedeki jinekolojik laparoskopik prosedürlerin çoğunda tanısal değerlendirme veya cerrahi yaklaşımlar için kullanılmaktadır. Operasyon esnasında uterusun mobilizasyonunu sağlayarak, operatörün anatomik yapılara (vasküler pedikül, ligament, vajinal duvarlar vb.) erişmesini sağlarlar. Uterin manipülatörler, tüm histerektomilerde özellikle de zor histerektomilerde, myomektomi, endometriozis, pelvik restorasyon ve infertilite operasyonlarında kullanışlı ve vazgeçilmez enstrümanlardır. Ticari amaç ile birçok uterin manipülatör geliştirilse de günümüzde halen ideal bir uterin manipülatör bulunmamaktadır. Bu derlemede; uterin manipülatörlerin genel prensipleri ve uygulamaları hakkında bilgi verilmesi ve uterin manipülatörleri karşılaştıran literatürdeki sınırlı sayıdaki çalışmaların gözden geçirilmesi amaçlandı.

The key instruments of gynecological laparoscopy: Uterine manipulators

Abstract

Uterine manipulators are used for diagnostic assessment or surgical interventions in most of the advanced laparoscopy-assisted gynecological procedures. They ensure the mobilization of the uterus making it possible for the operator to access anatomical structures (vascular pedicle, ligament, vaginal wall, etc.). The uterine manipulators are useful and indispensable instruments for all hysterectomies especially in difficult hysterectomies, myomectomies, endometriosis cases, pelvic restoration, and the infertility surgery. While many uterine manipulators have been commercially developed, there is currently no ideal manipulator found. Here in this review, the aim is to give information about general principles and applications of uterine manipulators and to review the limited number of studies comparing uterine manipulators.
