Portsite Metastasis in Coincidental Gallbladder Tumors: A Case Report

Abstract

Carcinoma gall bladder is a disease with high mortality and is usually diagnosed as an incidental case among patients undergoing laparoscopic cholecystectomy for symptomatic gall stone disease. A 80-year-old woman underwent a laparoscopic cholecystectomy for symptomatic cholelithiasis. Twenty-four months later, she was admitted to the hospital with a complaint of a mass at the trocar site. A biopsy from the portsite led to the diagnosis of adenocarcinoma metastasis. A 56-year-old man was diagnosed as gallbladder carcinoma by postoperative histological examination following laparoscopic cholecystectomy. He underwent the second surgery of resection of gallbladder bed on the liver and portsite with lymph node dissection. That could raise a suspicion of malignancy of gallbladder and so use of retrieval bag as a routine measure in cases with bile spillage may reduce the incidence of portsite metastasis..

Rastlantısal Safra Kesesi Tümörlerinde Trokar Yeri Metastazı: Olgu Sunumu

Öz

Safra kesesi adenokarsinomu yüksek mortaliteye sahip bir hastalıktır. Genellikle semptomatik safra taşı hastalığı için laparoskopik kolesistektomi geçiren hastalar arasında tesadüfen tanı konur. Seksen yaşında bayan hastaya semptomatik kolelitiazis nedeniyle laparoskopik kolesistektomi uygulandı. Yirmi dört ay sonra trokar yerinde bir kitle şikayeti ile hastaneye başvurdu. Trokar yerinde alınan biyopsi ile adenokarsinom metastazı tanısı kondu. Elli altı yaşında erkek hastanın laparoskopik kolesistektomi sonrası histopatolojik incelemesi ile safra kesesi adenokarsinom tanısı kondu. Uygulanan ikinci cerrahi müdahale ile karaciğerde safra kesesi yatağı ve trokar yeri rezeksiyonu ile birlikte lenf nodu diseksiyonu yapıldı. Laparoskopik kolesistektomide karsinom şüphesi olduğu düşünülen olgularda safra kesesi endobag ile karın dışına alınmalıdır. Böylece trokar yeri metastaz sıklığını azaltacaktır.
