Comparison of Iliac and Femoral Autogreft Practices in Salter and Pemberton Pelvic Osteotomies

Abstract

Objectives: To evaluate the radiographic results of iliac and femoral autologous graft using in Salter pelvik osteotomy (SPO) and Pemberton pelvik osteotomy (PPO). Materials and Methods: This study consist of 28 hips of 21 patients to that they SPO and PPO were applied for developmental displasia of hip (DDH). Patients were divided into different groups according to the type of graft, type of osteotomy and patient's age. Early and late neutral pelvic AP radiograms were evaluated. Effects of the graft type, osteotomy type and patient's age on the graft union time and loss of graft height were evaluated. The findings obtained from groups were evaluated by statistically with Mann-Whitney, Paired Samples T, Student's T tests. Results: No significant difference was found between SPO and PPO groups according to graft union time and loss of graft height (p>0.05). The age of operation time was no effect on loss of graft height and time of graft union (p>0.05). Although earlier union was found with iliac graft, there was no statistically significant difference between the graft types according to the union time (p>0.05). Height loss was significant in iliac graft (p0.05). Conclusion: Age of patient and type of osteotomy does not affects on the graft union time and loss of graft height. Especially, in patients with femoral shortening, if the age group is small, we recommend the use of femoral graft for prevent the loss of graft height.

Salter ve Pemberton Pelvik Osteotomilerinde İliak ve Femoral Otogreft Uygulamalarımızın Karşılaştırılması

Abstract

Amaç: Salter pelvik osteotomi (SPO) ve Pemberton pelvik osteotomi (PPO) uyguladığımız olgularda osteotomi hattına konan iliak ve femoral otogreftlerin, radyolojik sonuçlar üzerine etkisini literatür eşliğinde değerlendirmeyi amaçladık. Gereç ve Yöntem: SPO ve PPO yapılan 21 gelişimsel kalça displazili (GKD) hastanın 28 kalçası, bu çalışmaya dahil edildi. Hastalar, kullanılan greft tipine, osteotomi şekline ve yaşa göre kendi içerisinde farklı gruplara ayrıldı. Çalışmaya alınan hastaların erken ve geç nötral pelvis AP grafileri incelendi. Greft tipinin, osteotomi şeklinin ve hasta yaşının greft kaynama zamanı ve greft yükseklik kaybı üzerine olan etkileri değerlendirildi. Grup-lardan elde edilen veriler, istatistiksel olarak Mann-Whitney, Paired-Samples T, Student's T testleri kullanılarak değerlendirildi. Bulgular: Greft kaynama zamanı ve greft yükseklik kaybı açısından SPO ve PPO grupları arasında anlamlı bir fark bulunmadı (p>0.05). Ameliyat esnasındaki yaş, greft yükseklik kaybı ve greft kaynama zamanları üzerinde etkili değildi (p>0.05). İliak greft daha erken kaynasa da, kaynama zama-nı açısından greft tipleri arasında istatistiksel anlamlı fark yoktu (p>0.05). İliak greftte anlamlı yükseklik kaybı görülürken (p0.05) Sonuç: Hasta yaşı ve yapılan pelvik osteotomi şekli, greft kaynama zamanı ve greft yükseklik kaybı üzerine etkili değildir. Özellikle, femoral kısaltma yapılan hastalarda, yaş grubu da küçük ise, greft yükseklik kaybını önleyeceği için femoral greft kullanılmasını öneriyoruz.
