Our First Laparoscopic Achalasia Experience

Abstract

Achalasia is a motility disorder that reveals itself with an increase in lower esophageal sphincter (LES) pressure, loss of peristaltism at the lower end of esophagus and with lack of LES relaxation. Patients admit with the complaint of increasing dysphagia throughout years. Diagnosis is achieved by esophageal manometry and barium graphies. Choices of medical treatment are pneumatic dilatation, bouginage, botulinum toxin injections, nitrates and calcium channel blockers. Best effective treatment of achalasia is the ablation of lower esophageal sphincter. In order to prevent reflux caused by myotomy, fundoplication is frequently added to the operation. Laparotomy and lately laparoscopy took place of myotomy performed previously through thoracotomy, thus hospital stay is shortened, rapid healing is achieved and costs are able to be reduced. In his case report, we wanted to share our first laparoscopic Heller myotomy and Toupet partial posterior fundoplication experience.

İlk Laparoskopik Akalazya Deneyimimiz

Abstract

Akalazya alt özefageal sfinkter (AÖS) basıncının artması, sfinkter gevşemesinin bozulması ve özefagus alt ucunda peristaltizmin kaybolmasıyla kendini gösteren bir motolite bozukluğudur. Hastalar yıllar içinde artan yutma güçlüğü şikayeti ile başvururlar. Bunun sonucunda kronik gastroözefageal reflü ve buna bağlı komplikasyonlar ortaya çıkabilir. Teşhis, özefageal manometri, baryumlu özefagus grafileri ile konur. Medikal tedavi seçenekleri, pnömonik dilatasyon, bujinaj, botulinum toksini enjeksiyonu, nitratlar, kalsiyum kanal blokerlerinden oluşur. Akalazyanın en etkili tedavi yöntemi alt özefagus sfinkterinin cerrahi ablasyonudur. Miyotomi sonrası gelişen asit reflüsünü önlemek amacıyla sıklıkla fundoplikasyon eklenmektedir. Önceleri torakotomi ile uygulanan miyotominin yerini, laparotomi, daha sonra da laparoskopi almış ve böylece hastanede kalış süresi azalmış, hastaların erken iyileşmesi sağlanmış ve maliyet düşürülebilmiştir. Biz bu vaka sunumunda, ilk laparoskopik Heller miyotomi ve Toupet parsiyel posterior fundoplikasyon deneyimimizi paylaşmak istedik.
