Metastatic Renal Cell Carcinoma Mimicking Pleural Mesothelioma Radiologically

Abstract

Pleural metastasis caused by renal tumor may be rarely seen. Hypoxic respiratory failure was detected in a 49 years old man suffering from breath-lessness. In thorax computed tomography, there were diffuse pleural masses and pleural effusion on the right hemithorax and pleural masses on the left hemithorax. In abdominal ultrasonography, a mass lesion with a dimension of 8x7 cm, reported as a renal tumor. Tissue biopsy of right pleural mass using trucat needle revealed renal cell carcinoma metastasis. Closed tube drainage and talc pleurodesis on the right hemithorax was done.It was decided to follow the patient with symptomatic treatment because of the stage-4 disease and poor patient performance. The patient was dead at the third month of follow. The patient, dead because of the metastatic pleural masses of primary renal tumor and respiratory failure, was presented.

Radyolojik Olarak Plevral Mezotelyomayı Taklit Eden Metastatik Renal Hücreli Karsinom

Öz

Primer böbrek tümörlerine bağlı plevra metastazı nadiren görülmektedir. Nefes darlığı ile başvuran 49 yaşında erkek hastada hipoksik solunum yetmezliği tespit edilmiştir. Toraks bilgisayarlı tomografisinde sağda yaygın plevral kitleler ve plevral sıvı, solda plevral kitleler, batın ultrasonografisinde sol böbrekte 8x7 cm boyutunda böbrek tümörü ile uyumlu kitlesel lezyon izlenmiştir. Sağdaki plevral kitleden trucat iğnesi ile doku biyopsisi alındığında renal cell karsinom metastazı teşhisi koyulmuştur. Sağ hemitoraksa kapalı su altı drenajı ve talk plöredez uygulanmış, böbrek tümörü evresinin Evre-4 olması ve hastanın genel durumunun kötü olması nedeniyle semptomatik tedavi kararı alınmıştır. Hasta takibin üçüncü ayında kaybedilmiştir. Primer böbrek tümörüne bağlı metastatik plevral kitleler ve solunum yetmezliği ile kaybedilen hasta sunulmuştur.
