An Evaluation of Dapsone Therapy in Two Patients with Erythema Dyschromicum Perstans

Abstract

Erythema dyschromicum perstans (EDP) is a chronic, pigmentary disorder with symetrically distributed ashy macules on the face and neck, thrunk and proximal extremities. Early gray-blue macules with active red borders later become as gray-blue macules with indefinite edges. The etiology of EDP remains unclear, and it is proposed to be a reactive process to enviromental toxins or a manifestation of pigmentary allergic contact dermatitis. Therapy regimens such as sun protection, peeling lotions, antibiotics, topical and systemic corticosteroids, and vitamins have been reported to be ineffective whereas dapson therapy yielded hopefull results in various case reports. However, in our out-patient clinic, 38 and 53 years old, two female patients with EDP were treated with dapsone without any beneficial clinical outcome. Placebo-controlled clinical trials are needed to reveal out the effects of dapson on the early and late phases of EDP. ©2007, Firat University, Medical Faculty

Eritema Diskromikum Perstans Tanılı İki Olguda Dapsona Yanıtın Değerlendirilmesi

Abstract

Eritema diskromikum perstans (EDP), yüz, boyun, gövde ve ekstremitelerin proksimalinde, simetrik yerleşimli, kül renginde maküller ile seyirli, kronik bir pigmentasyon bozukluğudur. Erken dönemde, izlenen eritemli, deriden hafif kabarık aktif kenar bulunan gri-mavi maküller, geç dönemde kenarları belirgin olmayan gri-mavi maküller halini alır. Etyolojisi açıklık kazanmamış hastalığın, çevresel toksinlere karşı gelişen bir reaksiyon ya da pigmente alerjik kontakt dermatitin bir bulgusu olabileceği öne sürülmüştür. Kaşıntı, yanma, batma gibi belirgin yakınmalar oluşturmayan hastalık, kozmetik sorunlara neden olabilir. Tedavide, güneşten korunma, kimyasal soyucu ajanlar, antibiyotikler, topikal ve sistemik steroidler, vitaminler etkisiz bulunurken, dapson ile ümit verici sonuçlar bildirilmektedir. Kliniğimizde EDP tanılı 38 ve 53 yaşlarında iki kadın olgunun tedavisinde kullanılan dapson etkisiz bulunmuştur. Dapsonun, EDP'ın erken ve geç dönem belirtileri üzerine etkisinin değerlendirilmesi amacıyla plasebo kontrollü çalışmalara gereksinim vardır. ©2007, Fırat Üniversitesi, Tıp Fakültesi
