The Effects of Using Epidural Anesthesia with General Anesthesia on Plasma Levels of Cytokins and Cortizol in Patients with Lower Abdominal Surgery

Abstract

Objectives: During anesthesia management and at post-operative period it is necessary to prevent endocrine, metabolic and immunologic responses and also to protect hemodynamic stability. It is also a principal expectation to avoid side effects of drugs by using low doses. Our study aimed to search the effects of anesthesia method on endocrine and immunologic responses. Materials and Methods: Twenty three patients with atopic dermatitis whose age ranging from 8 to 35 (mean ± SE 21.77±2.88) and 23 healthy volunteer whose age ranging from 18 to 32 (mean ± SE 25±3.22) were obtained in Dermatology Department of Firat University Research Center. Alt Batın Cerrahisi Olgularında Genel Anesteziye Eklenen Epidural Anestezinin Plazma Sitokinleri Ve Kortizol Seviyelerine Etkisi

Abstract

Amaç: Anestezi uygulaması sırasında ve postoperatif dönemde hemodinamik stabilitenin korunabilmesi, endokrin, metabolik ve immünolojik yanıtların önlenmesi gerekmektedir. Düşük doz ilaç kullanımı ile yan etkilerinden kaçınılabilmesi de temel beklentidir. Çalışmamızda anestezi yönteminin endokrin ve immünolojik yanıtlar üzerine etkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Yerel etik kurul ve hasta onayı alınan elektif total abdominal histerektomi yapılacak olan 30 hasta (ASA I-II, 38-55 yaş) çalışmaya alındı. Premedikasyon uygulanmayan hastalar randomize olarak genel anestezi (Grup G, n=15), genel+epidural anestezi (Grup E, n=15) olarak iki gruba ayrıldı. Anestezi öncesinde, cilt insizyonundan 5 dak. sonra, anestezi başlangıcından 2 saat sonra ve 24 saat sonra olmak üzere 4 defa venöz kan örnekleri alındı. Bu örneklerden IL-1β, IL-4, IL-6 TNF-α ve kortizol ölçümü yapıldı. İstatistiksel yöntem olarak , gruplar arası değerlendirme için Mann Withney-U testi kullanıldı. Bulgular: Çalışmamızda iki grup arasında demografik değerlerin karşılaştırılmasında , IL-1β, IL-4, TNF-α plazma ölçümlerinde istatistiksel farklılık yoktu(p>0.005).Kortizol düzeyleri; anesteziden önce ve insizyondan sonra beşinci dakikada gruplar arasında anlamlı farklılık yoktu(p>0,05). Cerrahi müdahalenin 2. saat değerlerinde iki grup arasında istatistiksel olarak anlamlılık tespit edildi.IL-6 değerlendirilmesinde ilk ve ikinci ölçümde fark bulunmazken(p>0,05), cerrahi müdahalenin 2. ve 24. saatte iki grup arasında istatistiksel olarak anlamlılık tespit edildi. Sonuç: Alt batın cerrahisinde genel anesteziye ek olarak epidural anestezi uygulanması pro-inflamatuvar ve anti-inflamatuvar sitokinler üzerine belirgin bir etki göstermemiştir. Bulgularımız literatür sonuçlarıyla uyumlu bulunurken daha önceki araştırmalar ve bu çalışmadaki veriler değerlendirildiğinde inflamatuvar yanıtın yeterince açıklanamadığı, konunun tam olarak açıklığa kavuşması için daha ileri çalışmalara gerek olduğu görüldü. ©2005, Fırat Üniversitesi, Tıp Fakültesi
