A Rare Complication in the ICU: Missed Central Venous Guide Wire

Abstract

Percutaneous central venous catheterization (CVC) is an essential part of the care in the ICU, emergency department, and operating theater. Guidewires are routinely used in the Seldinger technique during central venous catheter placement that putting the catheter over a guide wire has established it is superior to other methods and is now the technique of choice thought the world. The use of guide wires appears to be innately safe in the hands of experienced practitioners. The accidental loss of an entire guide wire during central venous catheterization can result in serious patient harm and require additional investigations as well as retrieval procedures. An 88-year old female patient was admitted to the intensive care unit because of aspiration pneumonia, hypoxemic respiratory failure and bilaterally pleural effusion. She needed to put a CVC due to unable to put a peripherally vein way. A guide wire was unsuspectingly released and retained in the patient during the catheterization of the femoral vein is presented.

Yoğun Bakımda Nadir Bir Komplikasyon: Santral Vende Kalan Kılavuz Tel

Abstract

Perkütan santral venöz kateter (SVK) takılması yoğun bakım ünitesinde (YBÜ), acilde ve ameliyathanede hasta tedavisi için bir gereklilik haline gelmiştir. SVK takılması işleminde kateterin bir kılavuz tel üzerinden gönderildiği Seldinger tekniği rutin olarak kullanılmaktadır. Seldinger tekniğinin diğer yöntemlere üstünlüğü bilinmektedir ve dünya çapında tercih edilmektedir. Tecrübeli uygulayıcılarda kılavuz tel kullanımı güvenli olmaktadır. Kılavuz telin SVK takılması sırasında kaybolması hastaya ciddi zararlar verebilir, ek tetkiklere ve çıkarılma işlemine gereksinim duyulabilir. YBÜ’ne aspirasyon pnömonisi, hipoksemik solunum yetmezliği ve bilateral plevral efüzyon tanılarıyla kabul edilen,  periferik damar yolu takılamamasından dolayı SVK ihtiyacı olan, kateterizasyon işlemi sırasında kılavuz telin femoral vende kaldığı fark edilerek, küçük cerrahi işlemle çıkartılan 88 yaşında kadın hasta olgusu sunulmuştur.
