Kronik Boyun Ağrısı Olan Hastalarda Başın Anteriyor Tiltte Oluşuna Göre Ağrı, Servikal Bölge Özellikleri, Özür Düzeyleri ve Yaşam Kalitesinin Karşılaştırılması

Abstract

Amaç: Bu çalışma başta anteriyor tilti olan ve olmayan kronik boyun ağrılı bireylerin ağrı şiddeti, servikal bölge özellikleri, özür düzeyleri ve yaşam kalitesinin karşılaştırılması amacıyla yapılmıştır.
Gereç ve Yöntem: Kronik boyun ağrılı bireyler başta anteriyor tilti olan (n=43) ve olmayan (n=41) şeklinde iki gruba ayrılmıştır. Her iki gruptaki bireylerin ağrı şiddetleri, servikal bölge eklem hareket açıklığı, eklem pozisyon hissi hatası, servikal bölge yüzeyel kas kuvveti ve enduransı, derin servikal fleksörler kasların enduransı, özür seviyesi ve yaşam kalitesi değerlendirilmiştir. İki grup karşılaştırması için Fisher x2 Testi ve Bağımsız Gruplar T-Test kullanılmıştır.
Bulgular: Başta anteriyor tilti olan ve olmayan grubun yaş ortalamaları sırasıyla 45,97±11,52 ve 41,63±10,26 idi. Bağımsız Gruplar T-Test sonuçlarına göre, başta anteriyor tilti olan grubun ekstansiyon yönündeki eklem pozisyon hissi hatası daha yüksek; derin servikal fleksör kas enduransı ise daha düşüktü (p<0,05). Bununla birlikte, her iki grubun ağrı şiddeti, eklem hareket açıklığı, eklem pozisyon hissi hatası (ekstansiyon hariç), yüzeyel kas kuvveti ve enduransı, özür seviyesi ve yaşam kalitesi benzer bulunmuştur (p>0,05).
Sonuç: Kronik boyun ağrılı bireylerde başta anteriyor tilt gözlendiği durumlarda, eklem pozisyon hissi (özellikle ekstansiyon yönünde) ve derin servikal fleksör kasların enduransı detaylı olarak değerlendirilmelidir. Bu bireylerin tedavi programlarında eklem pozisyon hissini artırmaya ve derin servikal fleksör kasların eğitimine yönelik yaklaşımların yer alması kronik boyun ağrılı bireyler için faydalı olabilir.

Comparison of Pain, Cervical Region Characteristics, Disability Levels and Quality of Life According to Forward Head Posture in Patients with Chronic Neck Pain

Abstract

Objectives: The study was conducted to compare the pain intensity, cervical region characteristics, disability level and quality of life of individuals with chronic neck pain with and without forward head posture (FHP).
Materials and Methods: Individuals with chronic neck pain were allocated two groups as FHP (n=43) and non-FHP (n=41). Pain intensities, cervical range of motions, joint position sense errors, cervical region superficial muscle strength and endurance, endurance of deep cervical flexors muscles, disability level and quality of life of individuals were assessed for both groups. Fisher x2 Test and Independent Groups T-Test were used for comparison of two groups.
Results: The mean ages of FHP and non-FHP groups were 45.97±11.52 and 41.63±10.26, respectively. Independent Groups T-Test results indicated that the joint position sense error in the extension was higher; deep cervical flexor (DCF) muscles endurance was lower in FHP group (p<0.05). At the same time, pain intensities, range of motions, joint position sense errors (except extension), superficial muscle strength and endurance, disability level and quality of life of both groups were similar (p>0.05).
Conclusion: When observed FHP in chronic neck pain patients, joint position sense error (especially in extension) and the DCF muscles endurance should be assessed more detailed. It may be beneficial for individuals with chronic neck pain to include approaches to increase joint position sense and to train DCF muscles in the treatment programs of these individuals.
