Acute Effect of Combining Movement Rhythm with Individual Music Preferences on Gait Parameters in an Individual with Parkinson's Disease, Case Report

Abstract

The aim of this case study was to examine the effects of combining personal music preference with rhythmic auditory stimulation on acute gait parameters in an individual with Parkinson's disease. The patient was a 77-year-old male individual with mild Parkinson's disease (UPDRS: 19) without dementia and freezing during walking. There was no recent fall history in the case. The case was included in the study twice in one week, and he was asked to indicate his favorite music genre and a piece of that genre. On the first day of the case, the walking time and number of steps required for rhythm determination, as well as step lengths were measured. In the second interview, he walked first with rhythmic auditory stimulation and then with personal musical preference combined with rhythmic auditory stimulation and the results were measured again. Rhythmic auditory stimulation was positively effective in all parameters. The combination of rhythmic auditory stimulation and personal musical preference showed more effective results than rhythmic auditory stimulation alone. As a result of this case study, an acute improvement in gait was observed in an individual with mild Parkinson's disease. We think that a study with more individuals and more time should be done in order to determine the effectiveness of this approach fully.

Abstract

Bu olgu çalışmasının amacı kişisel müzik tercihi ile ritmik işitsel uyarımın birleştirilmesinin Parkinson hastalığı olan bir bireyde akut düzeyde yürüyüş parametreleri üzerindeki etkilerini incelemekti. Hasta 77 yaşında hafif düzeyde Parkinson hastası (UPDRS:19) demansı olmayan ve yürüyüşte donma izlenmeyen erkek bir bireydi. Olguda son dönemde düşme hikayesi gözlenmemiştir. Olgu bir hafta içinde iki defa çalışmaya alınmış olup kendisinden en sevdiği müzik türü ve bu türe ait bir eser belirtmesi istenmiştir. Olgu ilk gün ritim belirleme için gerekli olan yürüyüş süresi ve adım sayısı ile ayrıca adım uzunlukları ölçülmüştür. İkinci görüşmede ise ritmik işitsel uyarım ve kişisel müzik tercihi ile ritmik işitsel uyarımın birleştirilmiş haliyle yürümüş ve sonuçlar tekrar ölçülmüştür. Bütün parametrelerde ritmik işitsel uyarım pozitif anlamda etkili olmuştur. Ritmik işitsel uyarım ve kişisel müzik tercihin birleştirilmesi ise ritmik işitsel uyarımın tek başına kullanılmasına göre daha etkili bir sonuç göstermiştir. Bu olgu çalışmasının sonucunda hafif düzeyde Parkinson hastalığı olan bireyde yürüyüşte akut iyileşme görülmüştür. Bu yaklaşımın etkinliğinin tam olarak belirlenebilmesi için daha fazla birey ve daha fazla zamana yayılan bir çalışma yapılması gerektiğini düşünmekteyiz.
