BİPOLAR BOZUKLUK TANILI KADINLARDA GEBELİK VE DOĞUM SONU DÖNEM (Multidisipliner Yaklaşım ve Psikososyal Destek)

Abstract

Amaç: Bu çalışma; gebelik ve doğum sonu dönemde Bipolar Bozukluk tanılı kadınlarda biyolojik desteğin yanı sıra psikososyal desteğin de önemine ve multidisipliner yaklaşımın gerekliliğine dikkat çekmek amacıyla yapılmıştır. Gereç ve Yöntemler: Derleme makale olan bu çalışma, konuya ilişkin literatür taranarak hazırlanmıştır. Bulgular: Gebelik ve doğum sonrası bireylerin biyo-psiko-sosyal açıdan çoklu değişkenliğin yaşandığı uzun bir süreçtir. Bu karmaşık süreç, öncesinde de varsa; yeme bozukluğu, anksiyete bozuklukları, depresyon ve psikoz gibi psikiyatrik yakınmaların kısmen arttığı bir dönem olabilmektedir. BPB tanılı kadınların büyük bir kesiminin gebelik ya da doğum sonu dönemde bir duygu dalgalanması yaşadığı da dikkate değerdir. Farmakoterapinin yanı sıra psikososyal desteğin tedavi sürecine katkı sağladığı kanıtlanmış durumdadır. Bu nedenle BPB tanılı gebe ve lohusalarda rekürrens ve ilişkili etkenlerin tanılanması ve uygun tedavi planlanması önemlidir. Sonuç: Psikolojik tanılı, özellikle BPB tanılı kadınlarda gebelik ve doğum sonrası dönemde: yakın takip, sürecin doğru yönetimi, duygudurum değişikliklerinin gözlemlenmesi ve tedavinin takibi hayat kurtarıcı olabilir. Sağlık profesyonelleri tarafından yakın takipte olunmak, sağlıklı bir gebelik ve lohusalık süreci geçirmek, psikiyatrik bir sorunu olsun ya da olmasın tercihe bağlı olmayan zorunlu bir ihtiyaçtır. Bu nedenle kadınlara; koruyucu sağlığı da kapsayacak şekilde psikiyatri ve kadın sağlığı alanında görev yapan farklı disiplinlerin çalışma uyumunun gereği ve psikososyal desteğin önemi ortaya çıkmaktadır.

IN WOMEN DIAGNOSED WITH BIPOLAR DISORDER PREGNANCY AND POSTPARTUM PERIOD (Multidisciplinary Approach and Psychosocial Support)

Abstract

Objective: This study aims to draw attention to the significance of psychosocial support, as well as biological support, and the necessity of a multidisciplinary approach in women diagnosed with Bipolar Disorder during pregnancy and the postpartum period. Material and methods: The compilation article was created by reading through the literature related to this topic. This study, which is a compilation article, was prepared by scanning the literature on the subject. Results: Pregnancy and the postpartum period are long processes during which individuals experience biopsycho-social variability. If this complex process existed before, it might be a period when psychiatric complaints such as eating disorders, anxiety disorders, depression, and partial psychosis increased partially. It is also noteworthy that a large number of women diagnosed with BPD experience emotional fluctuations during pregnancy or the postpartum period. Along with pharmacotherapy, psychosocial support has been proven to contribute to the treatment. Therefore, it's important to identify recurrence and associated factors in pregnancy and postpartum women when BPD is diagnosed and to plan appropriate treatment. Conclusion: In women diagnosed with a psychological condition, especially BPD, during pregnancy and the postpartum period, close monitoring, proper management of the process, observation of emotional changes, and follow-up to treatment can be life-saving. Being closely monitored by health professionals
during a healthy pregnancy and postpartum period, whether or not you have a psychiatric problem, is a nonoptional necessity. It is, therefore, important for women to harmonize the work of different disciplines in the field of psychiatry and women's health, including preventive health, and the importance of psychosocial support.
