HEPATODİYAFRAGMATİK İNTERPOZİSYON: CHİLAİDİTİ SENDROMU: OLGU SUNUMU

Abstract

65 yaşında erkek olgu, kabızlık, göğüs ağrısı ve nefes darlığı semptomları ile kliniğimize başvurdu. Çekilen akciğer grafisinde sağ diyafragmanın yükseldiği ve hemen altında seviyelenmeler izlendiği görüldü. Bilgisayarlı toraks tomografisinde sağ diyafragma altında karaciğer üzerinde kolona ait haustrasyonlar görüldü. MRI incelemesinde diyafragmanın intakt olduğu saptanan olguya "Chilaiditi sendromu" tanısı konuldu. Laparatomi ile kolon serbestleştirildi. Gevşek olan diyafragmaya plikasyon uygulandı ve posteriora yer değiştirmiş olan karaciğer anatomik pozisyonuna getirildi. Postoperatif dönemde semptomları düzelen olgu 16 aydır izlenmekte olup, radyolojik olarak nüks saptanmamıştır.

HEPATODIAPHRAGMATIC INTERPOSITION: CHILAIDITI'S SYNDROME: CASE REPORT

Abstract

A 65 years old man with chronic constipation, chest pain and short of breath was admitted to our hospital. Elevation of the right hemidiaphragm was noticed on his Chest X-ray. Thorax computed tomography depicted haustrations between the right hemidiaphragm and the liver. MRI confirmed the intact hemidiaphragm. Therefore the patient diagnosed as a "Chilaiditi syndrome" by radiological. Laparotomy was performed. Colon was released from the adhesions and repositioned. Lose diaphragm was plicated and the liver was repositioned to its anatomic location. Postoperative period was uneventful and the patient was discharged. The patient has been on follow-up in 16 months without any radiological recurrence.
