MAZOT ASPİRASYONUNA BAĞLI PNÖMONİ

Abstract

Ateş yiyenlerin pnömonisi hidrokarbon ürünlerinin aspirasyonuna bağlı olarak gelişen, nadir görülen akut bir pnömoni tablosudur. Küçük çocuklarda, ateş yutarak gösteri yapan animatörlerde yada petrol ürünleri ile uğraşan kişilerde bu ürünlerin aspirasyonu sonucu ortaya çıkabilir. Hastaların klinik özellikleri, hastalığın seyri ve sonucu değişken olabilmektedir. Nefes darlığı, öksürük, hemoptizi, göğüs ağrısı, ateş, akciğer konsolidasyonları ve ardından lezyonların pnömotoseller oluşturarak kaviteleşmesi ile karakterizedir. Akut psödoenfeksiy öz bir durum olarak açıklansa da süperenfeksiyonlar eşlik edebilmektedir. Tedavide kortikosteroid ve antibiyotikler kullanılır. Burada mazot aspirasyonuna bağlı gelişen nadir görülen ve değişik radyolojik patern gösteren 50 yaşında pnömoni olgusu sunulmuştur. Olgumuzda klinik ve radyolojik tablonun başlangıçta hızlı ilerlemesi, konsolidasyon alanlarının yaygınlığı ve erime alanlarının oluşması, ampiyem gelişmesi dikkat çekicidir.

PNEUMONIA CAUSED BY FUEL OIL ASPIRATION

Abstract

Fire eater ‘s pneumonia, a rare form of acute pneumonitis, develops after aspiration of hydrocarbon products. Small children, animators making performance with fire-eating or the people dealing with petroleum products are under the risk of this pneumonitis as a result of aspiration of these chemicals. The patients' clinical features, duration of diseases and outcome of diseases may be different. It is characterized by symptoms such as dyspnea, cough, hemoptysis, chest pain, fever and lung consolidations which typically cavitate forming pneumatoceles. Despite the fact that it is a pseudoinfectious entity, superinfections may sometimes coexist. Corticosteroids and antibiotics are used in treatment. 50-year old male patient, a rare case of fire eater's pneumonia due to diesel oil aspiration, with unusual radiological patern is presented. In our case, rapid progression of clinical and radiological findings at the early phase of disease, diffuse consolidations, formation of lysis in some areas and development of empyema are striking.
