GEÇ TANI ALAN SWYER-JAMES-MACLEOD SENDROMU OLGUSU

Abstract

Swyer-James-MacLeod sendromu (SJMS) akciğerin tamamında veya bir parçasında görülen tek taraflı saydamlık artışıyla karakterize nadir görülen bir hastalıktır. İlk kez 1953 yılında Swyer ve James tarafından tanımlanmış, daha sonra MacLeod tarafından detaylandırılmıştır. Çocuklukta geçirilen viral bronşiolit veya pnömoniye sekonder, edinsel gelişim gösterir. Semptomlar çoğunlukla tekrarlayan solunum yolu enfeksiyonları ile birlikte çocukluk döneminde başlar. Bronşektazisi az olan veya olmayan hastalar erişkin yaşa kadar tanı almayabilir. 54 y kadın hastanın 3 yıldır KOAH tanısı ile tedavi gören, efor dispnesi, öksürük, balgam, hışıltılı solunum yakınmaları varmış. Hastanın öyküsünde semptomların çocukluğundan beri olduğu son 10 yıldır arttığı öğrenildi. Solunum sistemi muayenesinde solda skapula altında inspiratuar ralleri vardı ve solunum sesleri azalmıştı. Akciğer grafisinde sol hilusda küçülme, sol orta ve alt zonda saydamlık artışı izlendi. Yüksek resolüsyonlu bilgisayarlı tomografide (YRBT) sol akciğerde vasküler dallanmada belirgin azalma buna bağlı diffüz havalanma artışı, bazal segmentlerde bronşektazi ile uyumlu bulgular saptandı. Hastanın d- dimer düzeyi ve alfa-1 antitripsin düzeyi normal sınırlardaydı. Ventilasyon/perfüzyon (V/Q) sintigrafisi çekildi. Sol akciğerde lingulada ve alt lobda match defekt izlendi. V/Q sintigrafisi bulgularının da uyumlu olması ile SJMS tanısı konuldu. Havalanma artışının her zaman amfizem-KOAH anlamına gelmediği unutulmamalı, kronik pulmoner semptomları olan hastalar ileri yaşta da olsa ayırıcı tanıda SJMS akla getirilmelidir.

DELAY IN DIAGNOSIS OF A CASE WITH SWYER-JAMES-MACLEOD SYNDROME

Abstract

Swyer-James-MacLeod Syndrome (SJMS) is a rare condition characterized by unilateral hyperlucency of a part of or the entire lung which was first described in 1953 by Swyer and James and further detailed by MacLeod. It is considered to be an acquired disease secondary to viral bronchiolitis and pneumonitis in childhood. The onset of symptoms typically occurs during infancy or early childhood in association with frequent respiratory infections. Patients who have little or no bronchiectasis have minor symptoms or are asymptomatic and may, therefore, miss their diagnosis until adulthood. A 54 years-old female patient who was taking bronchodilator treatment for three years with the complaints of dyspnea on exertion, cough, sputum and wheezing, by diaognosis of chronic obstructive pulmonary disease (COPD). Carefull history showed that the symptoms had continuing since childhood and increased for 10 years. Physical exanination revealed inspratory crackles under scapula and breath sounds had decreased. Chest X-ray showed shrinking on left hilum, hyperlucency on left middle and lower zone. There was d istinctive decrease in vascular erbranching on left lung and diffuse increase in aeration, bronchiectasis on basal segments. The ddimer and alpha-1 antityripsin levels of patient were normal. Ventilation / perfusion (V/Q) scan was performed. Match defects were seen on lingula and left lower lobe of left. The patient was diagnosed as SJMS. It should not be forgotten that hyperlucency always did not means emphysema-COPD, in diferential diagnosis of patients with chronic pulmonary symptoms SJMS should be considered.
