PULMONER ARTERİYOVENOZ MALFORMASYON: İKİ OLGU NEDENİ İLE

Abstract

Pulmoner arteriovenöz malform asyonlar (PAVM); akciğerin nadir görülen damar anomalileridir. Hastalar genellikle semptomsuzdur. Ancak, sağdan sola şanta neden olup, nefes darlığına yol açabilir ya da kanama sonucu hemoptizi ve hemotoraksa sebep olabilirler. AVM'lerin %70'den fazlası konjenitaldir ve olguların büyük çoğunluğu Rendu-Osler- Weber sendromu ya da herediter hemorajik telenjiektazi (HHT) olarak bilinen otozomal dominant geçişli patoloji ile birlikte görülmektedir. Hastanemize başvuran iki olgunun çekilen posterior anterior (PA) akciğer grafisi ve toraks bilgisayarlı tomografisinde (BT) kitlesel görünüm olması üzerine ileri tetkik yapıldı ve PAVM saptandı. Nadir görülmesi ve akciğer tümörü ayırıcı tanısında yer alması nedeni ile litaratür bilgileri gözden geçirilerek sunuldu.

PULMONARY ARTERIOVENOUS MALFORMATION: TWO CASE REPORTS

Abstract

Pulmonary arterivenous malformation are rare vascular abnormalities of the lung. Altough most patients are asymptomatic, AVM's can cause right to left shunt and this can lead dyspnea, hemoptysia or haemothorax. The vast majority of pulmonary AVM cases are related to Rendu- Osler-Weber Syndrome. Two cases admitted to our hospital chest x-ray and thoracic CT scans were detected. In further investigations, pulmonary AVM was diagnosed. It is presented in the light of pertinent literature because it is rarely seen and take aplace in the differential diagnosis of lung tumors.
