PULMONER MALTOMA

Abstract

Kuru öksürük yakınmasıyla başvuran elliüç yaşında erkek hasta akciğer malignitesi ön tanısı ile kliniğimize yatırıldı. Fizik muayenesinde her iki hemitoraksta, alt zonlarda inspratuar raller mevcuttu, diğer sistem bakıları olağandı. Akciğer grafisi ve toraks bilgisayarlı tomografisinde (toraks BT) sağ akciğer orta lopta 2 cm çaplı, sınırları düzensiz, etrafında hava bronkogramları bulunan, yarı homojen dansite artımı ve bilateral multipl metastatik nodüller izlendi. Bronkoskopide endobronşiyal lezyon saptanmadı ve bronş aspirasyonu benign sitolojiydi. BT eşliğinde transtorasik ince iğne aspirasyon biyopsisi sonucu kuşkulu sitoloji olarak raporlandı ve doku biyopsisi önerildi. BT eşliğinde tru-cut biyopsi ile lezyona ulaşılamaması üzerine açık akciğer biyopsisi uygulandı. Patolojik inceleme sonucu mukoza ile ilişkili lenfoma (MALT lenfoma/ MALToma) veya Psödolenfoma ayrımı tam olarak yapılamayan hastada immunohistokimyasal boyama yapıldı ve MALToma olarak rapor edildi. Hastada başka organ tutulumu saptanmadı. Hasta tedavi amaçlı hematoloji bölümüne sevk edildi. Literatürler gözden geçirildiğinde, pulmoner MALToman ın nadir görülmesi nedeniyle, bu olguyu sunmayı uygun bulduk.

PULMONARY MALTOMA

Abstract

A fifty- three year old man with nonproductive cough was referred to our clinic. Bilateral inspiratory crackles on inferior zones were found at the physical examination. In chest roentgenogram and torax computer tomography (torax CT), at the right middle lobe, a 2 cm sized, half-homogeneous density with irregular boundary and air bronchograms arround was seen and there were bilateral multiple metastatic nodules. Endobronchial lesion was not seen at the fiberoptic bronchoscopy and aspiration cytology was benign. Transtorasic fine niddle aspiration biopsy FNAB under CT showed suspicious cytology. As the lesion was not reached by CT associated tru-cut biopsy, open lung biopsy was made. The histopathology was mucosa associated lymphoid tissue lymphoma or Pseudolymphoma, after immunohistochemistry, it was reported as MALToma. Systemic screening revealed no other organ involvement. The patient was sent to hematology clinic for treatment. As this tumor type in the lung is relatively rare, we want to present this case.
