NADİR BİR TRAKEAL STENOZ NEDENİ: WEGENER GRANULOMATOSİS

Abstract

Wegener granülomatozisi solunum sisteminin granülomatöz enflamasyonu ve küçük-orta çaplı damarları tutan nekrotizan vaskülit ile karakterize sistemik bir hastalıktır. Olguların hemen tamamına yakınında üst solunum yollarına ait patoloji saptanır. Wegener hastalığının trakeo-bronşial tutulumları ise ülserasyon, psödotümör ve stenoza yol açar. Semptomatoloji astımı taklit edebilir. Olgumuz 48 yaşında kadın olup 3 yıldır astım tanısı ile tedavi almaktaydı. Kliniğimizde yapılan solunum fonksiyon testlerinde akım volüm halkasının inspiratuar fanında düzleşme görülmesi üzerine üst havayolu obstrüksiyonu kuşkusu ile incelendi. Bronkoskopik incelemede subglottik stenoz saptanması ve P ANCA nın pozitif olarak raporlanması nedeniyle Wegener granülomatozisi tanısı aldı. Hastaya kliniğimizde trakeal dilatasyon uygulandı ve şifa ile taburcu edildi. Bu çalışmada trakeal stenozların nadir bir nedeni olan Wegener granülomatozisi nedeniyle subglottik bölgede darlık gelişmiş ve uzun süredir astım tedavisi almakta olan bir olgumuzu literatür bilgileri ışığında tartışmayı amaçladık.

A RARE CAUSE OF TRACHEAL STENOSIS: WEGENER GRANULOMATOSIS

Abstract

Wegener granulomatosis (WG) is a systemic disease characterized by granulomatous inflammation of respiratory system and necrotizing vasculitis of small and medium sized vessels. An upper respiratory tract pathology is detected almost all of the cases. Tracheobronchial involvement of WG causes ulceration, pseudotumor formation and stenosis. Symptomatology can mimic the asthma. Our case was 48 years old female treated as athma since 3 years. She was examined with suspicion of upper airway obstruction since a plateau at the inspiration phase of flow-volume curve was estabilished at our out-patient clinic. After the determination of subglottic stenosis at bronchoscopic examination and pANCA was reported as positive, she was diagnosed as WG. Tracheal dilatation was performed at our clinic and she was discharged with cure. Our aim was to discuss the case of subglottic stenosis due to WG a rare cause of tracheal stenosis and was treated as asthma for a long time.
