BILATERAL TRAUMATIC CHYLOTHORAX FOLLOWING BLUNT THORACIC TRAUMA: A CASE REPORT

Abstract

Chylothorax is a rare form of pleural effusion that can be associated with traumatic and nontraumatic causes. Mostly, it is attributed to injury or overstretching of major thoracic duct by fractures or other injury of neighboring thoracic spine. The management of chylotorax ranges from conservative treatment to surgical intervention. Optimal management and outcome for patients with chylotorax remain unclear. We present a seventy five year old man who presented with respiratory difficulty due to multipl rib fractures and bilateral chylothorax, which developed after a blunt chest trauma. The patient was successfully treated through a conservative approach with total parenteral nutrition, nothing by mouth, and bilateral chest tube drainage. The nature of pleural effusion developed following a blunt thoracic trauma is important in the management of trauma patients.

KÜNT TORAKS TRAVMASI SONRASI BÜLATERAL TRAVMATİK ŞİLOTORAKS: OLGU SUNUMU

Abstract

Şilotoraks, tr avmatik ve travmatik olmayan nedenlerle ilişkili olarak plevral efüzyonun nadir görülen bir formudur. Sıklıkla torasik vertebra komşuluğundaki fraktürler veya diğer yaralanmalara bağlı olarak duktus torasikusun gerilme veya yaralanması nedeniyle oluşur. Şilotoraks tedavisi konservatif tedaviden cerrahi müdahaleye kadar gidebilir. Optimal yöntem net değildir. Biz künt travma sonrası multipl kot fraktürü ve bilateral şilotoraks nedeniyle solunum zorluğu olan 75 yaşında bir erkek olguyu literatür eşliğinde sunduk. Hasta bilateral göğüs tüpü drenajı ve oral alım kesilerek total parenteral nutrisyon ile konservatif yaklaşımla başarılı bir şekilde tedavi edildi. Özellikle künt toraks travmasını takip eden plevral efüzyonu olan travma hastalarında, plevral efüzyonun içeriğini belirlemek tedavi yönetiminde çok önemlidir.
