SERUM PROKALSİTONİN DÜZEYİNİN AKCİĞER TÜBERKÜLOZUNUN TANISI VE TEDAVİ CEVABININ DEĞERLENDİRİLMESİNDEKİ ROLÜ

Abstract

Giriş: Prokalsitonin (PCT) seviyeleri sağlıklı insanlarda ölçülemeyecek kadar düşük seviyelerde iken, enfeksiyonlar sırasında yükselmektedir. Nonspesifik bakteriyel enfeksiyonlarda daha fazla yükseldiği tespit edilmiş olup tüberküloz (TB) hastalarında da PCT düzeyinin artmış olduğu saptanmıştır. Gereç ve Yöntem: Çalışmamızda toplum kökenli pnömoni (TKP) hastaları ile TB hastalarında tedavi öncesi ve tüberküloz hastalarında tedavi sonrası PCT seviyeleri ölçülerek karşılaştırılmıştır. Çalışmaya 19-82 yaş aralığında aktif akciğer TB tanısı alan HIV (-) 27 hasta ve klinik, radyolojik, laboratuvar bulguları ile pnömoni tanısı olan 31 hasta olmak üzere toplam 58 olgu dahil edildi. Bulgular: TB'de PCT seviyesinin tedaviyle anlamlı derecede düşüş gösterdiği, PCT' deki bu düşüşün yaştan ve cinsiyetten bağımsız olduğu, pnömoni gibi akut akciğer enfeksiyonlarında saptanan PCT düzeyinin TB gibi kronik akciğer enfeksiyonlarındaki PCT düzeyine göre anlamlı olarak yüksek olduğu saptandı. Sonuç: TB hastalarında serum PCT seviyesinin tek başına tanı koymada yetersiz olduğu, tüberküloz tedavisi ile PCT seviyesinde anlamlı düşüş olduğu ve tedaviye cevabın değerlendirilmesinde yardımcı bir takip kriteri olabileceği düşünüldü.

THE ROLE OF SERUM PROCALCITONIN LEVELS IN DIAGNOSIS AND RESPONSE TO TREATMENT OF PULMONARY TUBERCULOSIS

Abstract

Aim: Procalcitonin (PCT) levels are too low to measure in healthy individuals whereas during the infections levels began to increase. Higher levels are measured in nonspesific bacterial infections but also seen that its high in tuberculosis patients. Material and Methods: In our study PCT levels are measured in community acquired pneumonia and pulmonary tuberculosis before and after treatment and compared. 27 patients HIV negative active pulmonary tuberculosis and 31 patients with community acquired pnemonia total 58 patients (age 19 – 82 year) are enrolled to the study. Results: During pulmonary tuberculosis treatment PCT levels are seem to be significantly decreasing, this detected regression is independent from age and sex. İn acute lung infections such as pneumonia PCT levels are significantly higher than in chronic pulmonary infections such as tuberculosis (TB). Conclusion: As a result serum PCT levels are inadequate for the diagnosis of tuberculosis but with the tuberculosis treatment serum PCT levels are significantly decreasing in addition serum PCT levels can be helpful for assessment of response to tuberculosis treatment.
