Fonksiyonel endoskopik sinüs cerrahisi yapılan pediatrik nazal polipli hastaların retrospektif analizi

Abstract

Amaç: Nazal kavite polipleri (NP) sıklıkla yetişkinlerde görülse de iki yaşından sonra herhangi bir zamanda ortaya çıkabilir. Çocuklarda NP nadir görülmekte ve etiyopatogenezi tam olarak bilinmemektedir. Bu çalışmada, çocukluk yaş grubunda rastlanan NP’li hastaların demografik özellikleri, kliniği, postoperatif histopatolojisi ve nüks oranları incelenerek güncel literatürdeki veriler eşliğinde tartışılması amaçlanmıştır.
Gereç ve Yöntem: Çalışmaya 2015-2020 yılları arasında kulak burun boğaz kliniğinde pediatrik yaş grubunda (7-18 yaş) NP tanısı almış ve cerrahi uygulanmış 22’si erkek, 16’sı kız toplam 38 olgu dahil edildi. Hastaların dosyaları retrospektif olarak tarandı; demografik özellikleri, sistemik hastalıkları, postoperatif histopatolojisi ve nüks oranları kaydedildi.
Bulgular: Hastalarda en sık rastlanan semptom burun tıkanıklığı (%84,21) idi. Nazal poliplerin 10’u (%26,31) bilateral, 28’i(%73,68) unilateral idi. Unilateral poliplerin 15’i (%39,47) sfeno-etmoidal ve 13’ü (%34,21) antrokoanal polip olduğu görüldü. Unilateral nazal poliplerin 12’si (%42,85) sağ, 16’sı (%57,14) sol nazal kavite kaynaklı idi. Nazal polip nedeniyle opere edilen 38 hastanın 26’sına primer cerrahi, 12’sine revizyon cerrahi uygulanmıştı. Hastaların 3’ü (%7,89) astım, 1’i (%2,63) kistik fibrozis ve 1’i (%2,63) kartagener sendromu tanısı almıştı.
Sonuç: Pediatrik hasta grubunda NP nadir görülmekle birlikte burun tıkanıklığı, burun akıntısı gibi nazal semptomlarla gelen hastalarda NP akılda tutulmalıdır. Diğer nazal patolojilerin ayırıcı tanısının yapılması, NP’nin takip ve tedavisi için önemlidir. Anamnez, fizik muayene ve görüntüleme yöntemlerinin etkili kullanılması nazal kavitede polip tanısını kolaylaştırmakta ve endoskopik sinüs cerrahisinin son yıllarda artan güvenilir bir tedavi yöntemi haline gelmesiyle birlikte hastalarının etkin bir şekilde tedavi edilmesi mümkün olmaktadır.

Retrospective analysis of pediatric patients undergoing functional endoscopic sinus surgery with diagnosis of nasal polyp

Abstract

Objective: Although nasal cavity polyps (NP) are most commonly seen in adults, they can occur at any time after the age of two years. NP in children is rare and its etiopathogenesis is not known. In this study, demographic characteristics, clinic, postoperative histopathology and recurrence rates of NP patients in the childhood age group were analysed and discussed in the light of current literature.
Material and Methods: A total of 38 patients (22 boys and 16 girls) who were diagnosed with NP in the paediatric age group (7-18 years) and underwent surgery in the otorhinolaryngology clinic between 2015 and 2020 were included in the study. The files of the patients were retrospectively reviewed; demographic characteristics, clinic, postoperative histopathology, and recurrence rates were recorded.
Results: The most common symptom was nasal obstruction (84.21%).10 (26.31%) of the nasal polyps were bilateral and 28 (73.68%) were unilateral.15 (39.47%) of the unilateral polyps were spheno-ethmoidal and 13 (34.21%) were anthrochoanal polyps. Twelve (42.85%) and 16 (57.14%) of the unilateral nasal polyps originated from the right and left nasal cavity, respectively. Of the 38 patients operated for nasal polyps, 26 underwent primary surgery and 12 underwent revision surgery. Asthma was diagnosed in 3 patients (7.89%), cystic fibrosis in 1 patient (2.63%) and Carthagener's syndrome in 1 patient (2.63%).
Conclusion: Although NP is rare in paediatric patients, NP should be kept in mind in patients presenting with nasal symptoms such as nasal obstruction and nasal discharge. Differential diagnosis of other nasal pathologies is important for the follow-up and treatment of NP. Effective use of anamnesis, physical examination and imaging methods facilitates the diagnosis of polyps in the nasal cavity, and with endoscopic sinus surgery becoming an increasingly reliable treatment method in recent years, it is possible to treat patients effectively.
