ÖZOFAGUS CERRAHİSİ SONRASI GELİŞEN GASTROBRONŞİYAL FİSTÜL: İKİ OLGUNUN DEĞERLENDİRİLMESİ

Abstract

Gastrobronşiyal fistül (GBF) oldukça nadir görülen ve çoğu özofagogastrik cerrahiye (özellikle Nissen fundoplikasyon) bağlı ortaya çıkan bir komplikasyondur. Bu hastalar genellikle gastrik içeriği aspire ederler ve buna bağlı rekürren ateş, hemoptizi, bronşit, pnömoni, akciğer apsesi ve bronşektazi gibi pulmoner yangı tabloları gelişir. GBF sonucu gelişen pulmoner sepsisin mortalitesi çok yüksektir. Pnömonisi olan post özofajektomik hastalarda GBF’den şüphelenmek önemlidir. Erken tanı ve tedavi hayat kurtarıcıdır. Bu makalede özofagus malignitesi nedeniyle opere edilmiş ve rekürren pnömoni kliniği ile tedavi edildikleri sırada gastrobronşiyal fistül saptanan iki olgu literatür eşliğinde sunulmuştur.
