KONJESTİF KALPYETERSİZLİĞİ OLAN TİP 2 DİYABETİKLERDE GÜNDE TEK DOZ GLARGİNE İNSÜLİN TEDAVİSİ İLE GÜNDE 2 KEZ NPH İNSÜLİN TEDAVİSİNİN GECE HİPOGLİSEMİ GÖRÜLME SIKLIĞINA ETKİLERİNİN KARŞILAŞTIRILMASI

Abstract

Amaç: Tip 2 diyabetik hastalara kalp yetersizliği de eşlik ettiğinde gece hipoglisemisi görülme sıklığı artar. Çalışmamızda, bu grup hastalarda günde tek doz glargine insulin tedavisi ile günde 2 kez NPH insulin kullanılmasının gece hipoglisemi gelişmesi üzerine etkilerini karşılaştırmayı amaçladık. Gereç ve yöntem: Yaş, vücut kitle indeksi, diyabet ve kalp yetmezliği süreleri ve metabolik göstergeler açısından benzer özelliklere sahip hastalar glargine insulin ve NPH insulin tedavi gruplarına ayrıldılar. Hastalar 12 hafta izlendiler. Bulgular: Glisemik kontrolün glargine insulin uygulanan grupta, diğer gruptan belirgin olarak daha iyi sağlandığı gözlendi (p < 0,001). Gece hipoglisemi görülme sıklığı glargine insulin grubunda % 10,5, NPH grubunda % 9,1 olup istatistiksel olarak farklı değildi. Genel hipoglisemi oluşma sıklığı ise glargine insulin uygulananlarda fazlaydı (p= 0,029). Sonuç: Günde tek doz Glargine insulin kullanımı tip 2 diyabetes mellitusla beraber kalp yetersizliği olan hastalarımızda günde 2 kez NPH insulin kullanımına göre daha iyi kan şekeri kontrolü sağlamış olmakla birlikte gece hipoglisemi görülme sıklığını azaltmamıştır. Bu nedenle bu grup hastalarda uzun etkili insulin kullanımında gece hipoglisemisi gelişmesi olasılığı bakımından dikkatli olunmalıdır.
