AKSİYEL UZUNLUĞUN FAKOEMÜLSİFİKASYON CERRAHİSİ SONRASI ÖN KAMARA PARAMETRELERİ VE GÖZ İÇİ BASINCI DEĞİŞİMLERİNE ETKİSİ

Abstract

Amaç: Fakoemülsifikasyon ve göz içi lensi (GİL) implantasyonu sonrası ön kamara(ÖK) parametreleri, santral kornea kalınlığı (SKK) ve göz içi basıncı (GİB) değişimlerini ve aksiyel uzunluğun(AU) bu parametrelerdeki değişimlere etkisini incelemektir. Gereç ve Yöntem: Çalışmada fakoemülsifikasyon cerrahisi geçiren 89 hastanın 106 gözü retrospektif olarak değerlendirildi. Olgular; AU ölçümleri 21(49 göz) olan iki gruba ayrıldı. AU ölçümleri IOLMaster 500 ile yapıldı. Tüm olgulara fakoemülsifikasyon cerrahisi öncesi ve postoperatif 3. ayda; Pentacam HR ile ÖK derinliği (ÖKD), ÖK hacmi (ÖKH), ÖK açısı (ÖKA) ve SKK ölçüldü. Goldman aplanasyon tonometresiyle GİB ölçümleri yapıldı. Verilerin analizinde T-test ve tek-yönlü varyans analizi kullanıldı. p≤0,05 anlamlı kabul edildi. Bulgular: Grupların postoperatif 3. aydaki ÖKD, ÖKA, ÖKH,GİB değişimlerinin anlamlı olduğu görüldü (p<0,001 hepsinde). SKK artışı anlamlı bulunmadı (p=0,24). ÖKD (p=0,044) ve ÖKA (p=0,006)’nın 21diğer gruba göre daha fazla arttığı; ÖKH (p=0,16), SKK (p=0,87) ve GİB (p=0,89) değişimleri açısından gruplar arasında anlamlı farklılık olmadığı saptandı. Sonuç: Fakoemülsifikasyon ve GİL implantasyonu normotansif gözlerde anlamlı olarak ÖKD, ÖKH ve ÖKA’da artmaya, GİB’de azalmaya neden olur. AU≤23 mm olan gözlerde cerrahi sonrası ÖKD ve ÖKA daha fazla artmaktadır.

EFFECT OF AXIAL LENGTH ON ANTERIOR CHAMBER PARAMETERS AND INTRAOCULAR PRESSURE CHANGES AFTER PHACOEMULSIFICATION SURGERY

Abstract

Objective: To investigate the changes in anterior chamber (AC) parameters, central corneal thickness (CCT) and intraocular pressure (IOP) after phacoemulsification and intraocular lens
(IOL) implantation, and the effect of axial length (AL) on these parameters. Material and Method: In the study, 106 eyes of 89 patients who underwent phacoemulsification surgery were evaluated retrospectively. Patients were divided into two groups with AL measurements of 21(49 eyes). AL measurements were made with IOLMaster 500. All
patients were assesed preoperatively and postoperatively on the 3rd month. Pentacam HR was used to measure the depth of AC (ACD), volume of AC (ACV), angle of AC (ACA) and CCT. IOP
measurements were performed with a Goldman applanation tonometer. T-test and one-way analysis of variance were used for data analysis. p≤0.05 was considered significant. ≤23>Results: ACD, ACA, ACV and IOP changes were significant in the postoperative 3rd month (p<0.001 in all). The increase in CCT was not significant (p=0.24). ACD (p=0.044) and ACA (p=0.006)
increased significantly in the group with 21to the other group. There was no significant difference between the groups in terms of ACV (p=0.16), CCT (p=0.87) and IOP (p=0.89) changes. ≤23>Conclusion: Phacoemulsification and IOL implantation in normotensive eyes significantly increase ACD, ACV and ACA, and decrease IOP. In eyes with AU≤23 mm, ACD and ACA increased significantly after surgery.
