İNTÖRN HEKİM VE HEMŞİRELERDE İNME FARKINDALIĞI VE İNMELİ HASTAYA YAKLAŞIMIN ARAŞTIRILMASI

Abstract

Amaç: İskemik inme, hızlı ve doğru bir şekilde tanı konularak, gerekli tedavi için zamanla yarışılan acil bir durumdur. Bunun için inme semptomlarını bilerek hızlı tanı koymak, uygun transfer yapabilmek ve doğru ilk müdahale yapabilmek gerekir. Bu çalışmamızda, hastanemiz hemşireleri ve Tıp Fakültesi son sınıf öğrencilerinin inme semptomları, hastaya ilk müdahale, hasta transferi ve tedavi yaklaşımları hakkında bilgi ve farkındalıkları araştırılmıştır. Yöntem: Hastanemiz çalışanı 55 hemşire ve Tıp Fakültesi son sınıf öğrencisi 55 intörn hekim olmak üzere toplam 110 gönüllü katılımcıya, inme farkındalığı, FAST semptomları, ilk müdahale ve nakil bilgisi, inme merkezinde ilk tanı ve tedavi yaklaşımlarını değerlendirmek üzere 10 sorudan oluşan çoktan seçmeli test uygulanmıştır. Bulgular: Sonuçlar incelendiğinde, hemşirelerin inme farkındalık oranı %51,1 olarak saptanmıştır. İnmeli hastaya ilk müdahale olarak, hipogliseminin taranması gerektiği, iki damar yolunun açılması ve hipertansiyona müdahale edilmemesi gerektiği cevaplarına hemşirelerin %20,9 gibi oldukça düşük oranda cevap vermiş olmaları dikkat çekici olmuştur. İntörn hekimlerin doğru cevap oranı nispeten daha yüksek %77,6 olarak tespit edilmiştir. Hemşirelerin ve intörn hekimlerin verdikleri cevaplarda, yaş, meslekteki süre, inmeli hastaya müdahale etmiş olma ve inme eğitimi almış olma durumları açısından istatistiksel olarak anlamlı farklılık saptanmamıştır (p>0,05). Sonuç: Tüm sağlık çalışanlarına verilmesi gereken eğitimlerle, akut iskemik inme semptomlarının tanınması, ilk müdahale olarak parmak ucu kan şekerinin ölçülmesi, hipertansiyona müdahale edilmemesi gerektiği ve hastaya hızlı tanı konularak 4,5 saat içerisinde trombolitik tedavi alma şansının kaçırılmaması gerektiği bilgilerinin hatırlatılmasına ihtiyaç vardır

AN INVESTIGATION OF STROKE AWARENESS AND APPROACHES TO STROKE PATIENTS IN INTERN PHYSICIANS AND NURSES

Abstract

Objective: Ischemic stroke is an emergency, requires prompt and accurate diagnosis to determine the need for urgent hospitalization and specific treatment. This study investigates our hospital’s nurses’ and intern physicians’ knowledge and awareness of ischemic stroke. Method: A questionnaire consisting of 10 questions evaluating stroke awareness, the symptoms, first intervention, transportation and first treatment approaches, was completed by 110 volunteer participants (55 nurses and 55 intern physicians). Results: The rate of stroke awareness of nurses was 51.1%. In terms of knowledge concerning the first assessment of the patients, it was a remarkable result that the detection of hypoglycemia, opening two vascular access routes and not interfering with hypertension scored a very low rate of 20.9% in nurses. The correct answer rate of intern physicians to the questionnaire was 77.6%. Statistically, there was no significance between the responses of the nurses and the intern physicians in terms of age, occupational duration, stroke intervention and stroke training history (p>0.05). Conclusion: All healthcare professionals should be trained in the diagnosis of acute ischemic stroke, the necessity of measuring fingertip blood glucose, not to interfere with hypertension and understanding the value of thrombolytic treatment in the first 4.5 hours.
