EVRE 2 SUBAKROMİAL SIKIŞMA SENDROMUNDA MANYETİK ALAN TEDAVİSİNİN ETKİNLİĞİ

Abstract

Amaç: Bu çalışmanın amacı, toplumda sıkça karşılaşılan ve üretken nüfusu etkilemesi nedeniyle iş günü ve fonksiyon kaybına yol açan, evre 2 subakromiyal sıkışma sendromu (SSS)’nun tedavisinde bir konservatif tedavi yöntemi olan manyetik alan tedavisinin etkinliğini araştırmak idi. Gereç ve yöntem: Bu çalışmaya, anterior omuz ağrısı yakınımı olan ve manyetik rezonans görüntüleme tekniği ile evre 2 SSS tanısı konulan 40 hasta alındı. Hastalar randomize olarak üç gruba ayrılarak, üç hafta (15 seans) süresince, 1. gruba ultrason (US) (1,5 watt/cm2, pulse, 5 dakika, subakromiyal alana) + egzersiz, 2. gruba manyetik alan (MA) (1-6. seanslar arası 10 hertz frekans, 35 gauss alan şiddeti, 7.-15. seanslar arası 20 hertz frekans, 50 gauss alan şiddeti) + egzersiz ve 3. gruba sadece egzersiz uygulandı. Hastalara ağrı kontrolu için gerektiğinde parasetamol kullanmaları önerildi. Hastalar tedavi öncesi ve tedavi sonunda (üçüncü haftada), ağrı için vizuel analog skala, hareket açıklığı ölçümü ve Constant skorlaması ile değerlendirildiler. Bulgular: Tedavi sonuçları değerlendirildiğinde; ultrason uygulanan grupta, iyileşmenin diğer iki gruba oranla istatistiksel olarak anlamlı oranda daha fazla olduğu gözlendi. Sonuç: Manyetik alan uygulamasının, evre 2 SSS’nda ultrason ve egzersiz uygulamalarına göre bir üstünlüğü saptanmadı; ancak uygulama kolaylığı, yan etkisinin olmaması gibi avantajları bu fizik tedavi modalitesi üzerindeki araştırmaların yoğunlaştırılması gerektiği kanısını uyandırdı.
