ACNE VULGARİSLİ OLGULARDA PLAZMANİTRAT/NİTRİT DÜZEYLERİNİN İNCELENMESİ

Abstract

Amaç: Akne açık ve kapalı komedon oluşumu, eritemli papül ve püstüller, nodüller, psödokistler ve bazen de nedbe oluşumu ile karakterizedir. Acne vulgaris etyopatogenezinde sebum salınımında artış, duktal hiperkeratinizasyon (hiperkornifikasyon), foliküllere Propionibacterium Acnes kolonizasyonu ve inflamasyonun olduğu düşünülmektedir. Nitrik oksit (NO), derinin keratinosit, fibroblast, endotel, Langerhans ve mast hücrelerinde bulunan nitrik oksid sentaz (NOS) tarafından oluşturulan bir inorganik moleküldür. Sitokinlerin etkisi ile keratinosit ve mast hücrelerinde NOS indüklenmektedir. Oluşan NO guanilat siklazı stimüle ederek keratinosit ve mast hücrelerinde mitojenik etkili olan cGMP düzeylerinin artmasına ve böylece hiperkeratinizasyona neden olmaktadır. Bu çalışmanın amacı nitrik oksit düzeylerinin acne vulgarisin etyopatogenezindeki rolünü incelemekti. Gereç ve yöntem: Bu çalışmaya Acne Vulgaris tanısı konmuş 30 hasta ile, yapılan sistemik muayeneleri sonucu sağlıklı oldukları saptanan 16 kişi kontrol grubu olarak alındı. Gerek deney, gerekse kontrol grubunu oluşturan olgulardan sabah aç karına alınan venöz kan örneklerinde NO’in metabolik ürünleri olan nitrat/nitrit (NOx) düzeylerine Griess reaksiyonu ile bakıldı. Elde edilen sonuçlar Student’s t testi kullanılarak kıyaslandı. Bulgular: Deney grubunda nitrat/nitrit düzeylerinde kontrol grubuna göre istatistiki yönden anlamlı (p < 0,05) bir artış gözlendi. Sonuç: Sonuç olarak, akneli olgulardaki plazma nitrat/nitrit düzeylerindeki artışın bu bileşiklerin üretiminin hızlanmasına bağlı olduğu söylenebilir.
