ŞİŞMAN KADINLARDA METABOLİK RİSK GÖSTERGESİ OLARAK BOY UZUNLUĞUNUN DEĞERLERİ

Abstract

Bu çalışmanın amacı şişman kadınlarda boy uzunluğu ile metabolik risk faktörleri arasındaki ilişkiyi araştırmaktır. Çalışma kapsamına Şişmanlık polikliniğimize başvuran 1998 şişman(beden kitle indeksi BMI- > 27 kg/m2) kadın hasta alınmıştır (ortalama yaş 37.15±11.74yıl).Bu grupta açlık kan şekerinin 126 mg/dL, kolesterol ve trigliserid düzeylerinin 200 mg/dL, damar basıncının 140/90 mm Hg, ürik asit düzeyinin 5.5 mg/dL nin üzerinde ve HDL kolesterol düzeyinin 45 mg/dL nin altında bulunması risk göstergesi olarak kabul edilmiştir. Her risk faktörü varlığına | puan verilerek her hasta için toplam risk puanı hesaplanmıştır. Çalışma grubunda boy uzunluğu ile yaş (r=-0.2512, p<0.0001), maksimum damar basıncı (12-0.115/, p<0.0001), minimum damar basıncı (r=0.0952, p-0.0001), beden kitle indeksi (r=-0.2335, <0.0001), kolesterol (r=-0.1256, p<0.0001), glukoz (r=-0.0593, p=0.0093), bel/kalça oranı (r=-0.0543, p=0.0162) ve toplam risk skoru (r=-0.1183, p<0.0001) arasında anlamlı ve negatif yönde ilişkiler belirlenmiştir. Boy uzunluğu kolesterol, damar basıncı ve glukoz düzeyleri yüksek olanlarda norma! olanlardan anlamlı olarak düşük bulunmuştur. Şişman Kadınlarda kısa boyun çeşitli kardiyovasküler risk faktörleri ile birlikte olduğu anlaşılmaktadır. Bu gözlem boy üzerine etkili olumsuz intrauterin ve çocukluk çağı faktörlerinin erişkin yaşamda kardiyovasküler hastalık riskinde artışa yol açtığı hipotezini destekleyebilir.
