Cutaneous metastasis of epidermoid carcinoma of the larynx: A case report

Abstract

The frequency of distant metastasis of epidermoid carcinoma of the larynx is between 6.5-7.2%. The common sites of metastasis are lung, liver and bone. Cutaneous metastasis is very rare.a 56-year-old male with locally advanced larynx carcinoma who received curative radiochemotheraphy a year ago was presented to our department with subcutaneous nodular lesions, and fatigue. Physical examination revealed 0.5 cm sized, and 1x1cm sized subcutaneous nodular lesions in the right arm, and in the parasternal area, respectively. Histopathologic examination confirmed the skin metastasis of epidermoid carcinoma of the larynx. The patient has profound anemia, hypercalcemia, hypernatremia, and hyperglicemia with complicated diabetic foot. Although the symptomatic treatment began immediately, the patient died on the third day of hospitalisation. Cutaneous metastasis may be the first sign of local failure, of distant metastasis, or even of the undetected laryngeal carcinoma with poor prognosis.

Larinks kanserli cilt metastazlı bir olgu sunumu

Öz

Epidermoid larinks karsinomunda uzak metastaz yaklaşık %6.5%7.2 oranında görülmektedir. Sıklıkla akciğer, karaciğer ve kemiğe yayılım saptanmakla birlikte cilt metastazı oldukça nadir görülür. Bir yıl önce lokal ileri evre larinks kanseri tanısı ile kliniğimizde küratif radyokemoterapi yapıldıktan sonra takibe alınan 56 yaşındaki erkek hasta, Mayıs 2006 tarihinde genel durum bozukluğu nedeniyle başvurdu. Fizik muayenesinde, sağ üst kol medialinde 0.5 cm ve sternum sağ yanında 1x1 cm çapında, mobil, düzgün sınırlı iki adet subkutan nodül ve diabetik ayak saptandı. Göğüs duvarındaki lezyon eksizyonel biyopsi ile çıkarıldı ve mikroskopik olarak primer tümör ile uyumlu cilt metastazı olduğu görüldü. Hastanın cilt metastazı dışında lokal veya uzak tutuluma rastlanmadı. Rutin kan ve biyokimya tetkileri sonucunda, anemi, hiperkalsemi, hiperglisemi ve hipernatremi bulundu. Semptomatik tedaviye rağmen genel durumu kötüleşmeye devam eden hasta hastaneye yatırılışının 3. gününde kaybedildi. Cilt metastazı, uzak metastatik yayılımın, lokal bölgesel yinelemenin veya daha önce bulgu vermemiş larinks tümörünün ilk işareti olabilmektedir. Genellikle kötü prognoz göstergesi olduğu bilinmektedir.
