BEYAZ NOKTALANMA;YENİ BİR VULVOSKOPİK HUMAN PAPİLLOMAVİRUS HPV BULGUSU

Abstract

Vulvanın kolposkopla incelenmesi sırasında, özellikle labia minoraların iç kısmı ve forşette gözlemlediğimiz ve “beyaz noktalanma” olarak tanımladığımız görünümün nedenini araştırmak. Ocak 1999 ve Ocak 2002 tarihleri arasında, Süleymaniye Kadın Hastalıkları ve Doğum Eğitim ve Araştırma Hastanesi kolposkopi ünitesine başvuran 997 hasta içerisinden vulvalarında beyaz noktalanma olarak tanımladığımız görünüme rastlanılan 32 hastadan biyopsi alınarak histopatolojik inceleme yapıldı. Vulvada beyaz noktalanması bulunan 32 olgunun, vulvar biyopsi materyallerinin histopatolojik incelemesinde 26 olguda %81,3 Human Papillomavirüs HPV enfeksiyonu, 5 olguda %15,6 hipertrofik distrofi, 1 olguda %3,1 ise vulvar intraeitelyal neoplazi VIN ile birlikte HPV enfeksiyonu tespit edildi. Anormal transformasyon zonu ATZ nedeniyle serviksten alınan biyopsilerin histopatolojik muayenesinde 5 %15,6 olguda düşük evre servikal intra-epitelyal lezyon LGCIL , 1 %3,1 olguda ise yüksek evre servikal intra-epitelyal lezyon HGCIL tanısı konuldu. Çalışmamızda ortaya çıkan verilere göre vulvada ilk defa bizim tanımladığımız, özellikle labia minoraların iç yüzeylerinde ve forşette gözlemlenen beyaz noktalanma adını verdiğimiz görünüm sıklıkla HPV enfeksiyonuyla beraberdir. Beyaz noktalanma gözlendiğinde serviksin de kolposkopik olarak incelenmesi şarttır

“WHITE PUNCTIATION” A NEW VULVOSCOPIC HPV FINDING

Abstract

To investigate the reason for the apperance observed particularly on the inner surfaces of labium minus and fourchette and defined as “white punctuation” during colposcopic examination of vulva. Among 997 patients who were admitted to the colposcopy unit of Süleymaniye Maternity and Women’s Disease Research and Teaching Hospital, between January 1999 and January 2002, 32 patients with “white punctuation” were examined colposcopically and vulvar biopsies were obtained for histopatologic examination from inner parts of labium minus and fourchette. This appearance was not detected by naked eye but colposcopy revealed 0,2-0,5 mm dot shaped regularly spread, palpable lesions on the epithelial surfaces which absorbed acetic acid. After histopatologic examination of vulvar biopsy materials of 32 cases with white punctuation on vulva, in 26 cases %81.3 HPV infection, in 5 cases %15.6 hypertrophic distrophy, in one case %3,1 VIN and HPV infection were found. Histopathologic diagnosis was low grade cervical intra-epithelial lesion LGCIL in 5 %15,6 , high grade cervical intra-epithelial lesion HGCIL in 1 %3,125 among the colposcopy guided cervical biyopsies of cases with vulvar “white punctuation”. We reviewed the literature and classical colposcopy texbooks and could not find any definition of this lesion which we had defined and named as “white punctuation” Coexistance of these lesions with HPV is remarkable. According to the data obtained from our study, the appearance particularly observed in the inner surface of labia minors and fourchette ,which was first defined by us and named as “white punctuation “ is frequently seen with HPV infection.
