MEME KANSERİNDE SENTİNEL LENF NODUNUN İNTRAOPERATİF PATOLOJİK YANLIŞ NEGATİFLİĞİ

Öz

İntraoperatif sentinel lenf nodunun patolojik değerlendirilmesindeki yanlış negatiflik intraoperatif patolojik değerlendirme yöntemiyle negatif fakat postoperatif immünohistokimyasal incelemede pozitif saptanması sonucumuzu görmek ve bu hasta grubuna sonradan yapılan aksiller lenf nodu diseksiyonunda çıkan nonsentinal lenf nodlarının durumunu incelemek istedik. Araştırmamıza Ocak 2003 ve Mayıs 2010 tarihleri arasında Ege Üniversitesi Genel Cerrahi Kliniğinde tanı ve tedavi alan, klinik olarak aksillası negatif olan, erken evre meme kanserli 740 hasta dâhil edildi. Sentinel Lenf Nodu Biopsisi SLNB uygulanan 740 hastanın 18 %2.4 ’inde sentinel lenf nodu intraoperatif yöntemle negatif bulunduğu halde postoperatif immünhistokimya İHK ile pozitif saptanmıştır. 18 hastamızın tümüne aksiller diseksiyon uyguladık. Aksiller diseksiyon sonrası 4 %22 hastada non-sentinel lenf nodu NSLN tutulumu gözlenmiştir

PATHOLOGIC FALSE NEGATIVITY OF INTRAOPERATIVE SENTINEL LYMPH NODE IN BREAST NEOPLASM

Abstract

The aim of this study is to see the intra-operative sentinel lymph node pathologic assessment false negativity index and to evaluate the nonsentinel lymph node status after regular axillary lymph node dissection. Patients with early stages of breast cancer and clinically node-negative axilla admitted to Ege University General Surgery Department between January 2003 and May 2010 were included in this study. Immunohistochemistry evaluation revealed positive node in 18 2.4% out of 740 patient which were reported as node negative on intraoperative pathologic lymph node assessment. ALND is performed in 18 patients. We revealed NSLN involvement in 4 %22 patients after ALND
