PRİMER İNCE BAĞIRSAK TÜMÖRLERİ: ÇEYREK YÜZYILLIK KLİNİKOPATOLOJİK DENEYİM

Abstract

İnce bağırsak, gastrointestinal sistem mukozal yüzey alanının %90’ını, uzunluğunun %75’ini oluşturmasına rağmen, ince bağırsak tümörleri, tüm gastrointestinal tümörlerin %2’sinden azını oluşturmaktadır. Bu tümörlerin oldukça nadir görülmesi nedeniyle, ince bağırsak tümörlerinin epidemiyolojisine ilişkin veriler oldukça sınırlıdır. 1989-2014 yılları arasında batında kitle,subileus,ileus nedeni ile Hastanemizde ince bağırsak rezeksiyonu uygulanan 191 olgunun yaş, cinsiyet dağılımı, tümör yerleşimi ve tümörün histopatolojik özelliklerine ait veriler retrospektif olarak değerlendirilmiştir. Hastaların 73’ü %39 kadın, 118’i % 61 erkekti. Ortalama yaş 51.46 idi. Olguların %90’inde tümör primer olup, kalan %10’unda ise metastatik idi. En sık primer malign tümör adenokarsinomdu %62 . Daha sonra sıklık sırasına göre; lenfoma %14 , leiomyosarkom %6.5 , nöroendokrin tümör %6.5 ve diğerleri %11 görülmekteydi. Lokalizasyon olarak 159 olgu ince barsak jejunum-ileum , 34 olgu ise duodenum olarak saptandı. İnce bağırsakta primer adenokarsinomu olan 96 olgunun 11’inde eş zamanlı olarak kolonda ve 3 olguda ise yine eş zamanlı olarak mide adenokarsinomu mevcuttu. İnce bağırsak tümörleri İBT erkeklerde daha baskın, genelde ileri yaşta, ancak her dekatta görülebilmektedir. IBT az görülmekle beraber klinik olarak mutlaka akılda tutulması gereken bir hastalıktır. Tıpkı diğer gastrointestinal sistem tümörlerinde olduğu gibi senkron olarak da görülebilmektedir. Bu nedenle gastrointestinal sistemde ince bağırsakta tümör saptandığında, diğer gastrointestinal sistem taraması da yapılmalıdır. Sonuç olarak kliniğimizin 25 yıllık deneyimini paylaşmanın literatüre katkı sağlayacağı görüşündeyiz

PRIMARY SMALL INTESTINE TUMORS: A QUARTER CENTURY OF CILINICOPATHOLOGICAL EXPERIENCE

Abstract

Although the small intestine consists approximately 90% of the mucosal surface area and 75% of the total length of the gastrointestinal tract, tumors of small intestine account less than 2% of all gastrointestinal neoplasms. Due to the rarity of these tumors, data about the epidemiology of tumors is limited. We believe that sharing our series of 191 cases with rare prevalance among 25 years, will contribute to the literature. We retrospectively evaluated the data of age, sex, tumor location and histological features of 191 patients who underwent resection due to the mass of the small intestine between the years of 1989-2014 in our hospital. The gender of patients were as; 118 males 61% and 73 39% females. The median age of the patients was 51,46 years .We detected primary tumor in 172 %90 and metastatic tumor in 19 %10 of our case series. The most common histological type of tumor was adenocarcinoma %62 , followed by lymphoma 14% , carcinoid tumor 6.5% , sarcoma 6.5% and others 11% . In 159 cases, tumor was located in smallintestine jejunum-ileum , and in 34 cases in duodenum. Synchronous malignant tumor was determined in 14 of 96 patients with primary adenocarcinoma of small intestine. 11 of these cases were colon adenocarcinoma and 3 of them were gastric adenocarcinoma. In summary, tumors of small intestine are seen more common in men, generally in elderly, but may be seen in every decade. Although it is clinically rarely seen, tumors of small intestine always should be kept in mind. As seen in other tumors of the gastrointestinal tract, they can be seen as asynchronous tumors. Therefore, when a tumor is detected in the small intestine, a tumor scanning of total gastrointestinal system scanning should be performed
