İLK 33 HASTADA LAPAROSKOPİK BÖBREK CERRAHİSİ DENEYİMİMİZ

Öz

Laparoskopik böbrek cerrahisi günümüzde uygulamalardaki deneyimin artması sonucu gittikçe artan sıklıkta uygulanmaktadır. Bu çalışmada kliniğimizde renal kitle, non fonksiyone böbrek, pelvis renalis tümörü, böbrek kisti nedeniyle laparoskopik cerrahi uygulanan hastaların verilerini sunmayı amaçladık. Laparoskopik tamamlanan nefrektomilerden 2 %8 hastaya intraoperatif kan transfüzyonu yapıldı. Hastaların hepsi postoperatif birinci günde mobilize edildi. Hastaların drenlerini ortalama 2,6 günde alındı. Hastanede ortalama kalış süresi 3,4 gün idi. Sonuç: Laparoskopik cerrahi öğrenme eğrisi uzun olan bir cerrahi tekniktir. Sonuçlarının açık cerrahi teknikler ile uyumlu olması ve hastaların postoperatif dönemde yaşam kalitelerinin konvansiyonel yöntemlere göre daha iyi olması nedeniyle daha fazla tercih edilmeye başlamıştır

OUR LAPAROSCOPİC RENAL SURGERY EXPERIENCES IN FIRST 33 PATIENTS

Abstract

Laparoscopic renal surgery became more popular and applicable with the increasing experience. We aimed to evaluate the results of patients who were treated by laparoscopically because of renal mass, non-functioning kidney, tumors of pelvis renalis and renal cysts. Transperitoneal laparoscopic renal surgery was performed to 33 patients between March 2012 to December 2014 in our clinic. The mean age was 47 range:32-68 years. Laparoscopic left surgery was performed to 18 54% patients. Laparoscopic right surgery was performed to 15 46% patients. Radical nephrectomy was performed in 6 patients for renal masses, simple nephrectomy was performed in 17 51% patients for nonfunctioning kidney and radical nephroureterectomy was performed in 3 patients for urothelial tumors. Partial nephrectomy was performed in 1 patient who had double collecting system with upper pole hydronephrosis and laparoscopic cyst excision was performed in 6 18% patients. The mean operation time was 132 95-210 min in laparoscopic simple nephrectomy and 118 min in laparoscopic radical nephrectomy. Operation was converted to open surgery in 3 patients; the reason was bleeding in 2 patients and fibrosis in one patient. Intraoperative blood transfusion was performed to 2 8% nephrectomy patients. All of the patients were mobilized in postoperative first day. The drainage catheters were taken in 2,6 range: 1-4 days and the mean hospitilization time was 3,4 range: 2-6 days. Laparoscopic surgery is a surgical technique with a long learning curve. The pathological and surgical results were similar with open surgery. Laparoscopic surgery has began more preferred technique because of better quality of life.
