DOUBLE FİLTRASYON PLAZMAFEREZ İLE TEDAVİ EDİLEN OLGULARIMIZIN RETROSPEKTİF DEĞERLENDİRİLMESİ

Öz

Amaç: Plazmaferez, dolaşımdaki antikor, immün kompleks ve toksin düzeylerini düşürmek için kullanılan bir ekstrakorporeal yöntemdir. İmmün süreçlerin neden olduğu standart tedaviye yanıtsız hastalıklarda kullanılmaktadır.Double filtrasyon plazmoforezle ise daha çabuk yanıt alınmaktadır.Bu çalışmamızda double filtrasyon plazmaferez ile tedavi edilen olgu serimizi sunmak istedik. Yöntemler: Çalışmamızda Ağustos 2011-Nisan 2014 tarihleri arasında double filtrasyon ile plazmaferez DFP tedavisi alan 29 hasta alındı. Hastaların tanı anındaki demografik özellikleri, DFP tedavi endikasyonları, DFP tedavi sıklığı, DFP komplikasyonları, DFP tedavi yanıt durumları retrospektif olarak araştırıldı. Bulgular: DFP ile tedavi edilmiş 29 hasta alındı. Olgularımızın 19'u kadın %70.3 ,10'u erkek 29.7 olup min 15-max 75 yaş aralığında ortanca 46 yaş olduğu görüldü. Olguların tanıları incelendiğinde Myastenia Graves MG %44.8, Sistemik lupus eritamatozis SLE %10.3, Kalp Transplant Rejeksiyonu %6.9, Hiperlipidemi %6.9, Wegener Sendromu %6.9, Renal Transplant rejeksiyonu RTx %3.4, İsaac Sendromu %3.4, polinöropati %3.4, amiloidoz %3.4,Nöromiyaljia Optica NO %3.4, Multipl myelom MM %3.4, Gullain Barre Sendromu %3.4’dü. Hastaların 26sının %89.7 DFP yanıtının olduğu, 3 ünün %10.3 ise DFP yanıtsız olduğu tespit edildi. Üç hastanın hastalık progresyonu nedeni ile kaybedildiği, diğer 26 hastanın yanıtlı olduğu görüldü. Sonuç: Sonuç olarak DFP; özelikle immun süreçlerin neden olduğu belli hastalıklarda immunsupresif ajan, IVIG tedavilerine yanıtsız olduğunda tedaviye eklenebilir. DFP tedavisi ile erken yanıt ve daha az komplikasyon görülmektedir. DFP, standart plazmaferezin önüne geçmeye başlamıştır. Bu küçük olgu serimizde literatürü destekler sonuçlar elde edilmiş olup, literatür verilerine katkı sağlamak amaçlı sunulmuştur

RETROSPECTIVE EVALUATION OF PATIENTS TREATED WITH DOUBLE FILTRATION PLASMAPHERESIS

Abstract

Objective:Plasmapheresis is an extracorporeal method for reducing the level of circulating antibody , immune complexes and toxins. Plasmaphresis is used at the immune mediated disease of refractory to standart treatment.The double filtration plasmaphresis is to respond quickly.This study, we wanted to present our series of patients treated with double filtration plasmapheresis. Methods: In our study,29 patients were treated with double filtration plasmapheresis DFP between August 2011 and April 2014. The demographic characteristics of patients at diagnosis, treatment indications, DFP frequency, complications, response to treatment status were retrospectively evaluated.Results: 29 patients were treated with DFP.19 patients were female 70.3% ,10 were male 29.7% and the median age is 46. The diagnoses examined Myasthenia Graves MG 44.8%, Systemic Lupus Erythematosus SLE 10.3% Heart Transplant Rejection 6.9%, Hyperlipidemia 6.9%, Wegener's Syndrome 6.9%, Renal transplant rejection RTX 3.4%, Isaac Syndrome 3.4% polyneuropathy 3.4%, amyloidosis 3.4 %, Nöromiyalji Optica NO 3.4% Multiple myeloma MM 3.4% , Gullace Barre Syndrome 3.4 %. 26s of the patients 89.7% that DFP response, three of reputation 10.3% were found to be unresponsive to the DFP. 3 patients died of disease progression, response was observed in the other 26 patients.Conclusion:As a result, DFP may be added to immunosuppressive agents , IVIG treatment are the unanswered in particular diseases caused by immune processes. DFP is seen early response to treatment and fewer complications. DFP began to take precedence over the standard plasmapheresis. This small case series supports results were obtained in our literature, is presented to provide contributions to the literature
