PSÖDO MELANOZIS COLI: BEŞ OLGULUK SERİDE TEMEL HİSTOKİMYASAL ÖZELLİKLER

Öz

Melanozis coli, kolonoskopik incelemede koyu renkte görünüm ve patolojik incelemede ise lamina propriada kahve renkli pigment ile karakterize antitedir. Bunun, uzun süreli antrakinon türevi laksatif alımıyla ilişkili olabildiği, iskemik bağırsak hastalığı ve ülseratif kolit gibi hastalıklardaki mukozal görünümü taklit edebildiği de bildirilmektedir. Morfolojik olarak izlenen ilk bulgu, lamina propriadaki hücrede apoptozistir. Lamina propriada ölen bu hücreleri makrofajların fagosite etmesi sonrasında lipofuskin oluşur. Laksatif kullanım öyküsü olan, kolonoskopik ve/veya histopatolojik olarak tanı alan 5 hastayı sunuyoruz. H&E boyalı kesitlerin incelenmesine ek olarak tanımlanan pigmentlerin PAS + ve diastaza dirençli olduğu, Fontana-Mason boyasında da koyu kahve-siyah boyandı. Prusya mavisi demir boyasında söz konusu pigmentlerin boyanmadığı saptandı. Aldehid fuksin ve modifiye Ziehl-Neelsen boyası da uygulandı. Sonuç olarak Melanozis coli genellikle benign ve laksatif ajanın kullanımının durdurulması sonrasında geri dönüşümlü olabilen patolojik bir durumdur. Son yıllarda yapılan çalışmaların odak noktası olgularda kolorektal karsinom riskinin artıp artmadığı üzerinedir. Nadiren, tüm kolonun diffüz olarak melanozis coli tutulumunun psödoobstrüksiyon sebebi olabileceği de bildirilmektedir

PSEUDO MELANOSIS COLI: MAIN HISTOCHEMICAL FEATURES IN FIVE-CASES

Abstract

Melanosis coli is a condition characterized by the dark coloured appearence in colonoscopy and deposit of brown-black pigment in the lamina propria of the colon on pathologic examination. The condition is in association with chronic use of anthraquinone laxatives and can also mimic other diseases such as ulcerative colitis or ischemic bowel. Morphologically, the first finding is apoptosis of cells in lamina propria. Macrophages engulf these apoptotic cells in lamina propria and lipofuscin is formed. We share five cases who have a history of chronic laxative use and who diagnosed by colonoscopy and/or pathologic examination. This pigment was periodic acid-Schiff-positive and diastasis-resistant; also dark brown-black with Fontana-Masson stain. This pigment had no reaction with Prussian blue. Aldehyde Fuchsin and modified Ziehl-Neelsen were also performed. Finally, Melanosis coli is a pathologic condition that widely accepted as benign and reversible after stopping laxative use. Recently, all of the studies focus on the possible risk of colorectal carcinoma in these patients. Rarely, progression to diffuse involvement of the entire colon by melanosis coli may lead to pseudo-obstruction
