MENTAL RETARDE HASTALARDA ANTİKONVÜLSAN KULLANIMININ ROKURONYUM’UN NÖROMUSKÜLER BLOK ÖZELLİKLERİ ÜZERİNE ETKİSİ

Abstract

Bu çalışmada mental retarde hastalarda kronik antikonvülsan tedavinin rokuronyumun nöromusküler blok süresine etkisi geriye dönük olarak değerlendirilmiştir. Genel anestezi altında diş tedavisi yapılan ASA I-II 44 mental retarde hastanın verileri retrospektif olarak değerlendirildi. Kronik 3 aydan fazla antikonvülsan ilaç kullanan hastalar Grup 1, ilaç kullanmayan hastalar Grup 2 olarak adlandırıldı. Nöromusküler ileti monitorizasyonu TOF-Guard cihazı Organon Teknika, Denmark ile yapılmıştı. Anestezi indüksiyonu 2-2,5 mg/kg propofol, 0,6 mg/kg rokuronyum, 1µg/kg remifentanil ile sağlanmıştı. Anestezi idamesi %50 N2O/O2 karışımında %2 sevofluran konsantrasyonu ile sağlanmıştı. Hasta kayıtlarından rokuronyumun etki başlangıç süresi EBS , klinik etki süresi KES , derlenme indeksi Dİ ve TOF oranının % 75 T75 , %90 T90 derlenmesine dek geçen süreler kaydedildi. Hastaların demografik verilerinde istatistiksel anlamlı fark yoktu. EBS her iki grupta benzer bulundu Grup 1:104.27±11.15 sn, Grup 2: 103.45±10.92 sn . KES, Dİ ve TOF oranının % 75 T75 , %90 T90 derlenmesine dek geçen süreler Grup 1’ de istatistiksel anlamlı olarak daha kısa bulundu p

EFFECT OF ANTICONVULSANT USE ON THE NEUROMUSCULAR BLOCKING PROPERTIES OF ROCURONIUM IN MENTALLY RETARDED PATIENTS

Abstract

In this study, we retrospectively evaluated the effect of chronic anticonvulsant use on the neuromuscular blocking time of rocuronium in mentally retarded patients. The data of 44 mentally retarded patients of ASA I-II whose dental treatments were carried out under general anaesthesia have been evaluated retrospectively. The patients who used chronic more than 3 months anticonvulsant drugs were named as Group 1 and those who did not use them as Group 2. The neuromuscular transmission was monitored using the TOF-Guard device Organon Teknika, Denmark . The anaesthetic induction was enabled with propofol 2-2.5 mg/kg, rocuronium 0.6 mg/kg and remifentanil 1µg/kg. Anaesthesia was maintained with 2% sevoflurane + 50% N2O/O2inhalation. Rocuronium’s onset of action OA , duration of clinical action DCA , recovery index RI and the times elapsed until the TOF ratio recovered by 75% T75 and 90% T90 were recorded from patient records. There was no statistically significant difference in the demographic data of the patients. OA was similar in both groups Group 1: 104.27±11.15 sec. and Group 2: 103.45±10.92 sec. .
