TONSİLLEKTOMİ YAPILMIŞ OLGULARDA HİSTOPATOLOJİK AKTİNOMİKOZ İNSİDANSI

Abstract

Amaç: Bu çalışma ile beş yıllık süre boyunca kronik tekrarlayan tonsillit ve tıkayıcı tonsil hipertrofisi nedenleriyle tonsillektomi yapılmış olgularda histopatolojik olarak saptanan aktinomikoz insidansının araştırılması tanı ve tedavi yaklaşımlarının literatür eşliğinde gözden geçirilmesi amaçlanmıştır. Gereç ve Yöntemler: Bu çalışmada Antalya Atatürk Devlet Hastanesi ve İzmir Tepecik Eğitim ve Araştırma Hastaneleri Kulak Burun Boğaz Kliniklerindeki kronik tekrarlayan tonsillit ve tıkayıcı tonsil hipertrofisi tanısı ile tonsillektomi uygulanan 150 olgu dahil edildi. Tonsillektomi ile alınan dokular hematoksilen-eozin ile boyandıktan sonra ışık mikroskobu ile aktinomikoz varlığı değerlendirildi. Bulgular:Sağlık Bakanlığına ait iki hastane KBB kliniklerinde Ocak2010-Aralık 2014 tarihleri arasında kronik tekrarlayan tonsillit ve tıkayıcı tonsilhipertrofisi nedenleriyle tonsillektomi ameliyatı yapılan ve yaş dağılımı 3-26 yıl olan 150olgu retrospektif olarak incelendi.Tonsil dokusunun tamamı histopatolojik olarak değerlendirildi.Tonsilleraktinomikoz görülme sıklığı % 13 olarak bulundu. Sonuç:: Sadece malignite şüphesi olan olgularda değil, tonsillektomi uygulanan tüm olgularda cerrahi örneklerin histopatolojik incelemelerinin yapılmasının, hem çocukluk yaş grubunda nadir görülen tonsil hastalıkları erken tanısı ve insidansının belirlenmesinde, hem de kronik tonsillitetyolojisindeaktinomikozun rolünün belirlenmesinde katkısı olacağı düşüncesindeyiz

INCIDENCE OF HISTOPATHOLOGIC ACTINOMYCOSIS IN CASES WITH TONSILLECTOMY

Abstract

Objective: In this study, we aimed to investigate the detection rate of histopathologically confirmed actinomycosis in chronic recurrent tonsillitis and obstructive tonsillar hypertrophy tonsillectomized cases, during a 5-year period. Diagnosis and treatment approach is over viewed with the literature. Methods: 150 patients who underwent tonsillectomy for in chronic recurrent tonsillitis and obstructive tonsillar hypertrophy reasons in theDepartment of Otorhinolaryngology, Antalya Ataturk State Hospital and Izmir Tepecik Training and Research Hospital were investigated retrospectively. The surgical materials of all cases were evaluated histopathologically. Results: In two Ear Nose Throat clinics of Ministry of Health between January 2010 and December 2014 150 patients with the age distribution of 3 - 26 years undergoing tonsillectomy surgery for in chronic recurrent tonsillitis and obstructive tonsillar hypertrophy reasons were analyzed retrospectively. Tonsil actinomycosis of tonsillar tissue were evaluated histopathologically. Incidence of tonsil actinomycosis was found to be 13%. Conclusions: Weare of the opinion that the histopathological examination of surgical examination will contribute both to the diagnosis and incidence of rarely-seen chilhood tonsillar diseses and tothe role of chronic tonsillitis etiology of actinomycosis not only in patients with suspected malignancy but also in all those apllied tonsillectomy
