A case of pneumothorax due to non-invasive mechanical ventilation

Abstract

Emphysema is enlargement of alveolus, alveolary ducts and destruction of alveolary wall. One of complications of non-invasive mechanical ventilation (NIMV) is barotrauma of damaged lung. Here we present a 75 years old male who had Chronic Obstructive Pulmonary Disease (COPD), emphysema for 5 years and suffered from pneumothorax after NIMV. During treatment with NIMV his general condition deteriorated and oxygen saturation decreased immediately. Chest X-ray and tomography revealed pneumothorax. Chest tube inserted under local anesthesia. Although NIMV might seem like innocent, in patients whose general condition immediately worsens, oxygen saturation decreases, has emphysema and bullous lesions pneumothorax must be excluded. Noninvazif mekanik ventilasyon sonrası gelişen iyatrojenik pnömotoraks olgusu

Öz

Amfizem, alveol ve alveoler duktusların anormal genişlemesi, alveol duvarının dekstrüksiyonu ile karakterize bir hastalıktır. Noninvazif mekanik ventilasyonun (NIMV) komplikasyonlarından biri de özellikle hasarlı akciğerlerde oluşan barotravmadır. Burada 75 yaşında Kronik Obstrüktif Akciğer Hastalığı (KOAH) tanısıyla 5 yıldır takipli olan NIMV sonrası pnömotoraks gelişen bir olgu sunulmaktadır. Hastaya KOAH, solunum yetmezliği nedeniyle yoğun bakım ünitesinde NIMV uygulanırken genel durumunda aniden kötüleşme, oksijen saturasyonunda düşme saptandı. Çekilen akciğer grafisi ve tomografide pnömotoraksla uyumlu bulgulara rastlandı. Hastaya lokal anestezi altında toraks tüpü takıldı. NIMV masum bir işlem gibi görünse de KOAH\'lı hastaların genel durumunda aniden kötüleşme oksijen saturasyonunda düşme saptandığında pnömotoraks ekarte edilmelidir.
