Serebral palsili olgularda görme ve işitme bozuklukları ve epilepsi sıklığı

Abstract

Amaç: Bu çalışmanın amacı serebral palsili (SP) vakalarda işitme, görme problemleri ile epilepsi sıklığının saptamaktır. Gereç ve yöntem: Çocuk nöroloji polikliniğinde serebral palsi tanısı ile takip edilen ve hastaneye başvuran hastaların dosyaları retrospektif olarak incelendi. Bulgular: Bu çalışmada, %63.3\'ü erkek, %36.7\'si kız çocuğu olmak üzere toplam 460 vaka araştırıldı. Çalışmaya dahil edilen hastaların erkek/kız oranı 1.7 ve %60\'ı matür, %40\'ı ise prematüre doğumlu idi. Vakaların gösterdikleri klinik tablolara göre dağılımları; %39.7 spastik kuadriparezi, %28.7 spastik diparezi, %18.2 spastik hemiparezi, %7 koreoatetoik SP, %3.5 mikst tip SP, %1.1 hipotonik SP, %0.9 spastik monoparezi, %0.4 distonik SP, %0.4 ataksik SP olark saptandı. Serebral palsiye eşlik eden bozukluklar olarak %54.3\'ünde epilepsi, %38.5\'unda şaşılık ve %22\'sinde işitme kaybı saptandı. Sonuç: Serebral palsi kalıcı daha çok motor işlevlerle ilgili bir hastalık olarak algılansa da sıklıkla görme ve işitme problemleri ve epilepsi gibi ikincil sorunlara neden olmaktadır. Yüksek oranda tespit ettiğimiz eşlik eden bozuklukların erken tanı ve tedavisi hastaların hayat kalitesini olumlu yönde etkileyecektir.

Visual and hearing disabilities and epilepsy frequency in children with cerebral palsy

Abstract

Objectives: The aim of this study was to evaluate visual and hearing disabilities and epilepsy frequency in children with cerebral palsy. Materials and methods: The medical records of patients who were followed up with the diagnosis of cerebral palsy in Child Neurology Department were examined retrospectively. Results: In this study, Totally 460 patients of which 63.3% male and 36.7% female were included. Male/female ratio was 1.7 and 60% of the patients were mature and 40% of them were premature. The distribution of the cases according to clinical description was as follows: 39.7% spastic quadriplegia, 28.7% spastic diplegia, 18.2% spastic hemiplegia, 7% choreoathetoid cerebral palsy, 3.5% mixed type cerebral palsy, 1.1% hypotonic cerebral palsy, 0.9% monoparesis, 0.4% dystonic cerebral palsy and 0.4% ataxic cerebral palsy. Disorderss coexisting with cerebral palsy were epilepsy in 54.3%, strabismus in 38.5%, and hearing loss in 22% of patients. Conclusion: Although cerebral palsy is known as a disease with motor disability, brain pathology frequently causes hearing and visual disturbances and epilepsy. Early diagnosis and treatment of these accompanying disabilities will affect positively quality of life of these patients. J Clin Exp Invest 2012; 3(2): 245-249
