A case of hyperamylasemia: Macroamylasemia? Review of the literature

Abstract

Amylase is one of the analyzed tests in patients evaluated for abdominal pain. Macroenzyme disorder, a benign condition without any requirement for any treatment is among the reasons of the high serum enzyme levels. Macroamylasemia is the most frequently encountered macroenzyme disorder. Herein, we present a patient with abdominal pain and persistent high amylase levels considered as macroamylasemia. Establishing the diagnosis of macroenzyme disorder as the cause of the unexplained high enzyme levels prevents unnecessary, expensive and even invasive diagnostic and therapeutic procedures.

Abstract

Karın ağrısı ile başvuran hastalarda istenen tetkiklerden biri de serum amilazıdır. Serum amilaz enzim yükseklik nedenlerinden birisi de selim seyirli ve herhangi bir tedavi gerektirmeyen makroenzimlerdir. Makroenzimlerin en sık görüleni makroamilazemidir. Yazımızda karın ağrısı ve sebat eden amilaz yüksekliği ile başvuran makroamilazemili bir vaka sunulmaktadır. Açıklanamayan enzim artışının nedeni olarak makroenzimin tespit edilememesi pahalı, gereksiz, sıklıkla invaziv işlemlerle alternatif tanıların araştırılmasına ve cerrahi gibi agresif tedavilere neden olabilir.
