Is Newborn Jaundice a Risk Factor for the Wheezy Child?

Abstract

Objective: Many perinatal risk factors such as mother’s age and premature birth have been identified for childhood asthma and wheezy child. Some observational studies have shown a relationship between childhood asthma and hyperbilirubinemia and/or phototherapy. We aimed to investigate the relationship between neonatal jaundice and the incidence of wheezing and other respiratory tract diseases in the first year. Material and Method: Term newborns (n=254) who were admitted to the primary NICU between January 2017 and December 2018 due to the indirect hyperbilirubinemia also who received phototherapy, and healthy newborns (n=254) who were without anomalies and hyperbilirubinemia as control group were included in the study. İnfants in both groups were evaluated for 12 months in terms of respiratory diseases and wheezing attacks. The clinical and demographic data obtained from patient records and the type/frequency of respiratory diseases were confirmed by calling the families. Results: Of the patient’s group, 142 (55.9%) were male, the mean birth week was 38.07±1.57 and 158 (62.2%) were delivered by cesarean section. The total bilirubin level was 15.26±2.66 mg/dl at hospitalization and 8.20±2.51 mg/dL at discharge. The mean phototherapy time was 2.91±1.83 days. In the one-year follow-up, of the patients, 94 (37%) had wheezing, 54 (21.2%) had only upper respiratory tract infections, and 6 (2.3%) had other lower respiratory tract problems. Infants who were diagnosed as wheezy child had significantly higher phototherapy time than infants without wheezing (p<0.05). There were 71 (%27.9) children diagnosed as wheezy child in the control group, and there was a significant difference between the two groups (p<0.05). Conclusion: The history of taking phototherapy with jaundice in the neonatal period may be a risk factor for wheezy child development. Largescale studies are needed on this subject.

Yenidoğan Sarılığı Hışıltılı Çocuk İçin Bir Risk Faktörü Müdür?

Abstract

Amaç: Hışıltılı çocuk ve süt çocukluk çağı astımı için anne yaşı ve prematüre, sezaryen doğum, antibiyotik kullanımı, maternal ve paternal astım gibi çeşitli perinatal risk faktörleri bildirilmiştir. Bazı gözlemsel çalışmalarda çocukluk çağı astımı ile hiperbilirubinemi ve/ veya fototerapi arasında bir ilişki olduğu gösterilmiştir. Bu çalışmada yenidoğan sarılığı ile ilk bir yaşta hışıltılı atak, solunum yolu hastalığı görülme sıklığının arasındaki ilişkinin araştırılması hedeflenmiştir. Gereç ve Yöntem: İndirek hiperbilirubinemi tanısı ile Ocak 2017-Aralık 2018 tarihleri arasında I. düzey yenidoğan yoğunbakım ünitesine yatırılan ve fototerapi uygulanan term yenidoğanlar (n=254) ile kontrol grubu olarak çocuk sağlığı ve hastalıkları polikliniğine başvuran ancak yenidoğan döneminde hiperbilirubinemisi olmayan bebekler (n=254) çalışmaya alındı. Her iki gruptaki bebekler 12 ay boyunca solunum yolu hastalığı, hışıltılı atak görülme durumu açısından değerlendirildi. Hasta kayıtlarından ulaşılan klinik ve demografik veriler, solunum yolu hastalığı tipi ve sıklıkları aileler telefonla aranarak teyit edildi. Bulgular: Hasta grubundaki bebeklerin 142’si (%55,9) erkekti, doğum haftası 38,07±1,57 olup, 158’i (%62,2) sezaryenle doğmuştu. Serum total bilirubin düzeyi hastaneye yatışta 15,26±2,66 mg/dl, taburculuk anında 8,20±2,51 mg/dl idi. Ortalama fototerapi süresi 2,91±1,83 gün idi. Bir yıllık izlemde hastaların 94’ü (%37) 3 ve daha fazla hışıltılı atak geçirdi ve 54’ünde (%21,2) sadece üst solunum yolu enfeksiyonu, 6’sında (%2,3) diğer alt solunum yolu problemleri saptandı. Hışıltılı çocuk tanısı alan bebeklerin fototerapi alma süresi tanı almayan bebeklerden anlamlı düzeyde daha yüksek bulundu (p<0,05). Kontrol grubunda hışıltılı çocuk tanısı alanların sayısı (n=71, %27,9) hasta grubuna göre anlamlı olarak daha düşüktü (p<0,05). Sonuç: Yenidoğan döneminde sarılık ile birlikte fototerapi alma öyküsü hışıltılı çocuk gelişimi için bir risk faktörü olabilir. Bu konuda geniş çaplı çalışmalara ihtiyaç duyulmaktadır.
