Opsoklonus-Myoklonus Sendromu

Abstract

Opsoklonus-myoklonus sendromu OMS her yöne kaotik göz hareketleri, gövde ve ekstremitelerde myoklonik atımlar, ataksi ve davranış değişikliği olarak tanımlanan ender bir bozukluktur. Patogenezi tam olarak açıklanamamış olmakla birlikte, otoimmunite en sık suçlanan faktördür. Çocukluk çağında nöroblastom NB ile anlamlı birlikteliği söz konusu olup; NB’li çocukların %2-3’ünde OMS saptanırken, OMS’li çocuklarda NB görülme oranı %50-80’dir. On dokuz aylık hasta tarafımıza yürümede bozukluk ve ellerde titreme yakınmalarıyla başvurdu. Hastanın intensiyonel tremoru ve geniş tabanlı ataksik yürüyüşü vardı. Hastanın ataksi etiyolojisine yönelik tetkikleri normal sonuçlandı. Birinci haftanın sonunda klinik tabloya vertikal göz hareketleri, her dört ekstremitede görülen myokloniler eklendi. OMS malignite birlikteliği nedeniyle yinelenen batın USG’de 30x19 mm boyutlarında kitle saptandı. Nöroblastom ön tanısı ile alınan biyopsi iyi difarensiye tip nöroblastom olarak raporlandı. NB tedavisinde bağımsız olarak hastaya OMS’ye yönelik IVIG tek doz ve ACTH gün aşırı tedavileri başlandı. Bir hafta sonra klinik tabloda belirgin iyileşme saptandı. Çocukluk çağında akut ataksinin en sık iki nedeni ilaç alımı ve postenfeksiyöz serebellittir. Klinik izlemin sürekliliği ve dikkatli yapılan fizik muayene ile tespit edilecek ek bulgular tanı açısından değerlidir. OMS pediatrik paraneoplastik sendromların prototipidir ve nöroblastom ile sıkı birlikteliği vardır. OMS tedavisinde pulse steroid, intravenöz immunglobulin ve ACTH tedavisi önerilmektedir. Ayrıca birincil tedavilere yanıt alınamaması halinde siklofosfamid tedavisi önerilmektedir. Sonuç olarak, OMS ile başvuran hastalarda eşlik eden nöroblastom açısından kesinlikle ileri tetkikler yapılmalıdır. Nöroblastom taramasında ultrasonografik görüntülemenin yeterli olmayabileceği akılda bulundurularak şüpheli olgularda manyetik rezonans görüntüleme ile ileri değerlendirme yapılmalıdır.

Opsoclonus-Myoclonus Syndrome

Abstract

Opsoclonus-myoclonus syndrome OMS is a rare disorder characterized by chaotic eye movements in all directions, myoclonic movements in the trunk and extremities, ataxia, and behavioral changes. Although its pathogenesis has not been fully explained, autoimmunity is the most frequently blamed factor. Significant association with neuroblastoma NB is mentioned in childhood; the incidence of NB in children with OMS is 50-80%, while OMS is detected in 2-3% of children with NB. Our 19-month-old patient was presented to us with complaints of gait disorder and trembling hands. The patient had intentional tremor and broad-based ataxic gait. The patient’s examination results for ataxia etiology was within normal limits. At the end of the first week, vertical eye movements were added to the clinical situation, and myoclonus was observed in all four extremities. A mass of 30x19 mm was detected in abdominal ultrasonography US which was repeated due to the association of OMS with malignancy. Biopsy specimen obtained with the initial diagnosis of neuroblastoma was reported as welldifferentiated neuroblastoma. Independently from NB treatment, IVIG single dose and ACTH once in two days treatments for OMS were initiated. A significant improvement was noted in the clinical situation after one week. The most common two causes of acute ataxia in childhood are drug intake and postinfectious cerebellitis. The continuity of clinical follow-up and additional findings to be determined by careful physical examination are valuable in terms of diagnosis. OMS is a prototype of pediatric paraneoplastic syndromes and is closely associated with neuroblastoma. Pulse steroid, intravenous immunoglobulin and ACTH therapy are recommended in OMS treatment. Cyclophosphamide treatment is also recommended if response to primary therapies can not be achieved. In conclusion, further investigation should be done for the neuroblastoma accompanying the patients with OMS. In the case of suspicious cases, further evaluation should be done by magnetic resonance imaging considering that ultrasonographic imaging may not be sufficient in neuroblastoma screening
