Hidrops Fetalis ile Birlikte Nedeni Bilinmeyen Doğumsal Şilotoraks

Abstract

Doğumsal şilotoraks 10.000-15.000 doğumda bir görülür ve hidrops fetalisin ender nedenlerinden biridir. Bununla birlikte doğumsal şilotoraks yenidoğanda ve fetusta plevral sıvı birikiminin en sık nedenidir. Doğumsal şilotoraks hid- rops fetalis ile birlikte olduğunda ölüm oranı oldukça yük- sektir. Doğumsal şilotoraks tek başına olabileceği gibi lenfatik sistemin diğer bozuklukları, doğumsal kalp hasta- lıkları, Down, Turner, Noonan sendromu, pulmoner lenfan- jiektazi, kistik higroma ve H tipi trakeoözefageal fistül gibi diğer bozukluklarla birlikte de olabilir. Burada hidrops fetalis ile birlikte nedeni bilinmeyen doğumsal şilotoraks tanısı alan bir vaka sunulmuştur. Hidrops fetalisli bebekler- de ayırıcı tanıda doğumsal şilotoraks akla gelmelidir. Hidrops fetalis ile birlikte olan doğumsal şilotoraksın erken tanısı ve tedavisi bu bebeklerin sağ kalım oranlarını arttıracaktır

Idiopathic Congenital Chylothorax Presented with Hydrops Fetalis

Abstract

Although congenital chylothorax is a rare cause of hydrops fetalis with an estimated prevalence of 1/10.000-15.000, it is the most common type of pleural effusion in fetus and newborn. If congenital chylothorax is diagnosed in foetal life with hydrops fetalis, it has high mortality rates. Congenital chylothorax may occur alone. Hovewer it has been also reported with other lymphatic anomalies, conge- nital heart diseases, Turner, Down and Noonan syndromes, congenital pulmonary lymphangiectasia, cytic hygroma and H-type tracheo-oesophageal fistula. Herein, we report a case of idiopathic congenital chylothorax presented with hydrops fetalis. Chylothorax should be considered in the differential diagnosis of newborns with hydrops fetalis. Early diagnosis and treatment of congenital chylothorax with hydrops fetalis may improve the survival rates of the these newborns
