Çocuk Hematoloji/Onkoloji Hastalarında Vankomisine Dirençli Enterokokların İnfeksiyon/Kolonizasyon Sıklığı, Risk Faktörleri ve Klinik Sonuçlar

Abstract

Amaç: Vankomisine dirençli enterokoklar VDE özellikle immun suprese hastaların uzun süre yatarak tedavi gördüğü hematolojionkoloji ünitelerinin ciddi sorunu haline gelmiştir. Bu çalışmada; VDE kolonizasyonu ve infeksiyonu gelişen vakaları tanımlamak, risk faktörlerini ve kolonize hasta sıklığını belirlemek, infeksiyonda tedavi yaklaşımını ve klinik sonuçları değerlendirmek amaçlanmıştır. Gereç ve Yöntemler: Ocak 2007-Haziran 2010 tarihleri arasında İ. Ü. İstanbul Tıp Fakültesi, Çocuk Hematoloji-Onkoloji Bilim Dalı ile Çocuk Kemik İliği Transplantasyon Ünitesi’nde yatarak tedavi gören ve kültür örneklerinde VDE üremesi olan 32 vaka çalışmaya alındı. Vakaların klinik ve laboratuvar bulguları, tedavi ve izlem sonuçları retrospektif olarak değerlendirildi. Bulgular: VDE üremesi olan vakaların yaş ortalaması 106.7±64.6 ay 5 ay ile 216 ay olup, % 56’sı erkekti. Otuz iki hastanın 26’sı çocuk hematoloji-onkoloji, 6’sı KİT ünitesinde yatan hastalardı ve 14’ü ALL hastasıydı. En önemli iki risk faktörü olarak santral kateter % 93,8 ile yoğun kemoterapi ve buna bağlı immünsüpresyon % 90,6 saptandı. Vakalara son 3 ayda en sık kullanılan antibiyotikler amikasin, teikoplanin, meropenem, sefepim ve siprofloksasindi. En sık üreme rektal sürüntü örneğinde 25 hasta izlendi. Dokuz hastada VDE infeksiyonu gelişti. Kolonize olan ALL hastalarının % 14’ünde, AML ve KİT hastalarının % 50’sinde VDE infeksiyonu gelişmişti. VDE saptanan vakaların 22’sine linezolid tedavisi uygulandı. Hastaların 8’i % 25’i eksitus oldu. Ölen hastalara eşlik eden en sık infeksiyon akciğer mantar infeksiyonu 6/8 idi.Sonuç: Bağışıklığı baskılanmış hastalarda VDE önemli nozokomiyal patojenlerden biridir. VDE tek başına ciddi infeksiyon riski oluşturmasada, kateter, yoğun kemoteapi ve KİT gibi risk faktörlerinin varlığında morbidite ve mortalite riski artmaktadır. Riskli hastalarda VDE yayılımını ve mortaliteyi azaltmak için rektal sürüntüyüde içeren sürveyans çalışmaları yapılmalıdır

Frequency and Risk Factors for the Colonization and Infections of Vancomycin Resistant Enterococci VRE and Clinical Outcome in Pediatric Hematology/Oncology Patients

Abstract

Aim: VRE has become a serious problem especially in immunosuppressed patients in hematology-oncology clinics. In this paper we tried to define the VRE infected and colonized groups, determine its risk factors, and incidence, and evaluate treatment approaches to these infection and their clinical outcomes. Material and Methods: 32 patients who were admitted to pediatric hematology oncology department and bone morrow transplantation unit between January 2007 and June 2010 and had VRE colonization at culture samples were included in this study. We assessed the clinical, laboratory and treatment outcomes retrospectively.Results: 56 % of VRE colonized patients were male and they had a mean age of 106.7±64.6 months 5-216 months . Some 26 / 32 patients of them were admitted to hematology oncology department, and 6 of the cases were in-patients in the BMT unit and 14 of them were ALL. We determined the most important risk factors as central catheter 93.8 % , chemotherapy and immunosuppression 90.6 % . During the last 3 months of treatment, amikacin, teicoplanin, meropenem, cefepim, ciprofloxacin were the mostly used antibotics. The most common colonization sites were rectal samples 25 patients . 9/32 patients had VRE infection. 14 % of colonized ALL patients, 50 % of AML and BMT patients had WRE infection.Linezolid was used in for the treatm of 22 VRE patients, and 25 % of VRE infected patients expired. The most commonly concomitant infection in most patients was pulmonary fungal infection 6/8 . Conclusion: VRE is one of the most important nosocomial pathogens in immunocompromised patients, so good practice in antibiotic usage, isolation precautions and VRE surveillance studies are very important. VRE by itself is not a cause of serious infection, but together with the risk factors, morbidity and mortality might be higher. Surveillance studies including rectal sample cultures for risky patients to decrease VRE spread should be recommended
