Yenidoğanda Tuzlamaya Bağlı Gelişen Fournier Gangreni: Vaka Sunumu

Abstract

Fournier gangreni genellikle lokal travma, operasyon veya immün sistemi etkileyen hastalıklara bağlı gelişen, Gram pozitif mikroorganizmalar, enterik basiller ve anaeroblar tarafından oluşturulan skrotum, penis ve perineyi etkileyen akut enfektif bir gangrendir. Miadında 3,000 g ağırlığında, aralarında akrabalık olmayan sağlıklı anne-babadan doğan, 4 günlük erkek hasta yumurtalıklarında yara çıkması nedeni ile acil servisimize getirildi. Beyaz küre sayısı ve C-reaktif protein değerleri yaşına göre normaldi. Vaka Fournier gangreni tanısı aldı. Büyükanne tarafından çocuğun daha sağlıklı olması için tüm vücudunun tuzlandığı öğrenildi. Bu vaka sunumu Fournier gangreni ile karışabilen durumlara bölgemizde halen bir yanlış gelenek olarak devam eden tuzlama vakalarına dikkat çekmek ve öykünün halen ne kadar değerli bir parametre olduğunu vurgulamak amacıyla sunulmuştur.

A Case of Fournier’s Gangrene Associated With Salting in a Neonate

Abstract

Fournier’s gangrene is an acute infective gangrene of scrotum, penis and perineum caused by Gram positive microorganisms, enteric bacilli and anaerobic microorganisms which is generally associated with local trauma, operative procedures or diseases affecting the immune system. A 4-day old male patient, with a birth weight of 3000 g and born to nonconsangiunous healthy parents, was brought to the emergency department of our hospital with wounds on his scrotum. His white blood cell counts and C-reactive protein values were normal for his age. He was diagnosed as Fournier’s gangrene. It was ascertained that the grandmother salted the body of the child so that he could be healthier. Our aim in presenting this case is to place emphasis on the harmful tradition of baby-salting which still prevails in our region and may cause Fournier’s gangrene, and, importance of detailed history as an important diagnostic tool.
