Nötropeniye Yaklaşım

Abstract

Mutlak nötrofil sayısının 15000/mm3’ün altına düşmesi olarak tanımlanan nötropeni çocukluk çağında sık rastla- nan ve etiyopatogenezinde birçok mekanizmanın rol aldı- ğı bir tablodur. Konjenital ve akkiz nedenlerle ortaya çıkabilir. Klinik yelpazesi oldukça geniştir. Çok ağır ölümcül olabilen enfeksiyonlarla seyredebildiği gibi, semptom vermeden yalnızca laboratuvar tanısı almış vakalar da mevcuttur. Nedene bağlı olarak değişen klinik bulgular nedeniyle klinisyenler tanı koyma ve tedavi yak- laşımında bazen güçlükler yaşayabilmektedir. Nötropenik bir hastada değerlendirme yaparken ateşin öncelikle var- lığı dikkate alınmalıdır. Ateş varsa ayırıcı tanı için değer- lendirme yapmadan hızlı bir şekilde hastanın tedavisine başlamak sonrasında, detaylı değerlendirmeleri yapmak uygun yaklaşımdır. Bu yazıda nötropenik hastanın tanısı ve tedavi yaklaşımları üzerine derleme yapılmıştır

An Approach to Neutrophenia

Abstract

Neutropenia is defined as an absolute neutrophil count below 15000/mm3 . It is frequently encountered in pediatric age and many mechanisms can be involved in its etiopathogenesis. It can manifest as a congenital or an acquired entity. The clinical presentation can range from potentially fatal infections to asymptomatic laboratory findings. Because of varying symptoms dependent on etiologies, clinicians often have difficulty in diagnosis and management. The presence of fever priorly requires attention when evaluating neutropenia. When fever is present, commencing immediate treatment of the patient without making assessments for differential diagnosis and performing detailed evaluations is an appropriate approach This manuscript reviews the diagnostic, therapeutic approaches to neutropenia
