Adolesanlarda Anormal Uterin Kanamaya Jinekolog Gözüyle Yaklaşım

Abstract

Bu makale adolesanlarda görülen anormal uterin kanama- da güncel yaklaşımı ve yönetimi derlemektedir. Okuyucular orta ve şiddetli uterin kanamanın değerlendirmesi ve teda- visine pratik yaklaşımı bulacaklardır. Anormal uterin kana- ma adolesanlarda sık görülmekle birlikte yaşam kalitesini önemli miktarda bozan bir durumdur. Bu dönemde anovu- latuar sikluslar sık görülmekte ve anormal uterin kanama- nın en sık nedenini oluşturmaktadır. Değerlendirmede öyküye bakılmaksızın öncelikli olarak gebelik, travma ve cinsel yolla bulaşan hastalıklar dışlanmalıdır. Hafif kana- malarda yalnızca izlem yeterliyken akut ve hayatı tehdit edici kanamalarda yüksek doz kombine oral kontraseptifle- rin kullanımı, intravenöz östrojen verilmesi gerekebilmek- te, hatta dirençli durumlarda cerrahi girişimlere kadar gereklilik duyulabilmektedir

Gynecologic Approach to Abnormal Uterine Bleeding in Adolescents

Abstract

This article reviews the current understanding and mana- gement of abnormal uterine bleeding in adolescents. The readers have a chance to find an approach to the evaluati- on and treatment of mild to severe uterine bleeding. Abnormal uterine bleeding is a common problem which have significantly adverse effects on adolescents’ quality of life. In adolescence, most common underlying condition in abnormal uterine bleeding is anovulatory cycle. In the evaluation regardless of history, priorly pregnancy, trauma and sexually transmitted diseases must be ruled out. Although in the mild form of abnormal uterine bleeding the observation is enough, life threatening bleeding may neces- sitate use of high doses of combined oral contraceptives, intravenous estrogen or surgical intervention
